BIG cyst on lower back!
This is one of those blackheads that you were waiting to see! I thought this was a blackhead, or a Dilated Pore of Winer, but it proved to be an epidermoid cyst. It was flattened and disc-shaped because it's in a very tight area of the back and it couldn't grow down or up easily, so it spread out very flatly, which makes it look more like a large blackhead.

This is a lovely older patient whom I see for treatment of her skin cancers. I have a video I published earlier, that shows me extracting blackheads on her face, and it was during her last visit that I noticed that she had this cyst on the left side of her neck. However, we didn't have time to remove it on her previous visit. This time, I was seeing her to see how she was healing after I removed a basal cell carcinoma ( a type of non-life threatening skin cancer) and repaired the area surgically.
This bump had been bothering her for awhile, and I think she was happy to have it removed. As you know, epidermoid cysts like this are not life threatening. However, if they get irritated and inflamed, an infection can lead to pain and scarring. Here I use a punch biopsy tool to remove this cyst.

This patient has been seeing Dr Rebish recently because he has had this cyst on his neck which has recently become inflamed and “infected”. I put “infected” in quotes, because technically this is not an infection: When an epidermoid cyst ruptures, it creates a vigorous foreign body inflammatory response - this skin becomes warm, tender, painful, and swells, simulating an abscess. Incision and Drainage (I & D), will confirm the diagnosis of inflamed cyst, when the smelly, cheesy material is evacuated, and this often leads to rapid resolution of symptoms. These episodes are often misdiagnosed as “infection” of the cyst, but cultures are usually negative and antibiotic treatments is not required. Intralesional steroids can hasten the resolution of symptoms as well.
All this being said, we often prescribe antibiotics- it’s hard not to when you see something so red and inflamed and painful.. it seems almost cruel not to! Also, we as dermatologists, will just call the cyst infected, or call it an abscess. It’s more difficult to explain to patients why we would not call an area that looks angry and red and is painful non-infected. However, I felt I needed to clarify all of this in detail here, for medical education purposes!
Anyway, Dr Rebish had been injecting the area with steroids to decrease the inflammation but he had an appointment with me to finally excise the area because he was fed up with it. When I saw it, we prepared for an excision, but it actually turned into an I & D, the cyst was completely broken down because it had ruptured. After evacuating the area, there was a pretty large empty space which I packed with iodophorm gauze which may decrease the chance for continued inflammation or minimize chance of infection. I saw him a few days later because the area will still draining but was less swollen and less painful. At this point, I decided to pull out the rest of the gauze and just let it heal without it. I feel that the iodophorm gauze was not helping and perhaps worsening the situation. I wish I took a video of this, but I was in a hurry seeing him between patients, so I didn’t have the time! Sorry about that!
This may be a more disturbing video for many of you, and I will admit it was difficult for me to do. I don't like draining abscesses. They are ooey and gooey and often smell putrid, and you can see me carefully and gently squeeze and wipe away often. We are wearing splash screens. If this stuff gets in your hair or on your skin it can ruin your day. Especially when it gets in your hair, b/c it stays with you until the next time you take a shower. Luckily none of us get dirty, but this was my last patient before lunch and I had to go home and lie down for an hour or so to recover from this. Has to be done, but no. fun. The PATIENT on the other hand is lovely and I thank him for letting me treat him and film it for all of you to see.
At the end of this video, you can see that he was doing much better a couple of weeks after this procedure. No longer having pain or discomfort in the area, and in is minimally swollen and improving every day. We will continue to follow his progress and let you know if anything changes!

This patient has been seeing Dr Rebish recently because he has had this cyst on his neck which has recently become inflamed and “infected”. I put “infected” in quotes, because technically this is not an infection: When an epidermoid cyst ruptures, it creates a vigorous foreign body inflammatory response - this skin becomes warm, tender, painful, and swells, simulating an abscess. Incision and Drainage (I & D), will confirm the diagnosis of inflamed cyst, when the smelly, cheesy material is evacuated, and this often leads to rapid resolution of symptoms. These episodes are often misdiagnosed as “infection” of the cyst, but cultures are usually negative and antibiotic treatments is not required. Intralesional steroids can hasten the resolution of symptoms as well.
All this being said, we often prescribe antibiotics- it’s hard not to when you see something so red and inflamed and painful.. it seems almost cruel not to! Also, we as dermatologists, will just call the cyst infected, or call it an abscess. It’s more difficult to explain to patients why we would not call an area that looks angry and red and is painful non-infected. However, I felt I needed to clarify all of this in detail here, for medical education purposes!
Anyway, Dr Rebish had been injecting the area with steroids to decrease the inflammation but he had an appointment with me to finally excise the area because he was fed up with it. When I saw it, we prepared for an excision, but it actually turned into an I & D, the cyst was completely broken down because it had ruptured. After evacuating the area, there was a pretty large empty space which I packed with iodophorm gauze which may decrease the chance for continued inflammation or minimize chance of infection. I saw him a few days later because the area will still draining but was less swollen and less painful. At this point, I decided to pull out the rest of the gauze and just let it heal without it. I feel that the iodophorm gauze was not helping and perhaps worsening the situation. I wish I took a video of this, but I was in a hurry seeing him between patients, so I didn’t have the time! Sorry about that!
This may be a more disturbing video for many of you, and I will admit it was difficult for me to do. I don't like draining abscesses. They are ooey and gooey and often smell putrid, and you can see me carefully and gently squeeze and wipe away often. We are wearing splash screens. If this stuff gets in your hair or on your skin it can ruin your day. Especially when it gets in your hair, b/c it stays with you until the next time you take a shower. Luckily none of us get dirty, but this was my last patient before lunch and I had to go home and lie down for an hour or so to recover from this. Has to be done, but no. fun. This is one of those blackheads that you were waiting to see! I thought this was a blackhead, or a Dilated Pore of Winer, but it proved to be an epidermoid cyst. It was flattened and disc-shaped because it's in a very tight area of the back and it couldn't grow down or up easily, so it spread out very flatly, which makes it look more like a large blackhead.
As you can see, I was able to remove it entirely, and a couple sutures were placed.

The Story of 3 Pilar Cysts: Part 1 of 2

This woman came in accompanied by her physician husband, who has a practice in Southern California as well. She was recommended to see me by her son, who is in medical school and happened upon my videos on youtube. She has three pilar cysts, with two pretty large ones. She was SO kind to allow me to film the removal, and they were fun cysts to remove! I like excising pilar cysts, because the walls of these cysts are usually thicker and more durable so these cysts are more likely to pop out whole. This is ideal, because it usually ensures that we have removed the cyst in its entirety and that it will not likely reoccur.
In this video, I point out a distinctive sound that can be heard when doing surgery on the skin of the scalp. Under the skin and fat, just before you get to the bone of the skull, there is a thin area of tissue called the galea aponeurosis. When you tease and undermine away in this surgical plane, the tissue makes a distinctive noise.. see if you can hear it. Thought you may find this interesting. Also, this is the ideal surgical plane for closure. Although the scalp is very vascular and often bleeds, blood vessels don’t course through this surgical plane, so there is very low risk of bleeding when I am undermining the skin at this level.

A Pilar Cyst, aka a trichilemmal cyst, an isthmus-catagen cyst, or a “wen”, it looks identical on examination to the more common epidermoid cyst, except that 90% of them occur on the scalp. They tend to be more mobile and firmer than epidermoid cysts, and are derived from a different part of the hair follicle/skin. The internal content tend to be more “homogenous”/ smooth (looks different under the microscope), but it is also a benign growth. It is not uncommon to have multiple pilar cysts on the scalp and to have other family members to have these cysts (tend to run in families). Treatment is for cosmetic reasons usually (people don’t like the appearance of them), and excision with removal of the cyst and it’s contents is curative.

This particular pilar cyst excision will go down on My Favorites short list. This gentleman travelled from Hollywood to see me, and he was dressed to the nines, but very, very nervous. He admits he gets "white coat syndrome" and hates to see doctors. However, he saw me on The Doctors TV show, and he felt that I might be the one to remove this bump on his scalp.
As you can see he is pretty nervous but my favorite part was seeing his face right after I removed this cyst. He was smiling to himself and I could tell that he still couldn't believe that he went through with the surgery. He actually only had an appointment scheduled for a consult, but I could sense that he really wanted this bump removed and would probably chicken out and never come back, so luckily I had time enough to do it right then and there!
Also, there are parts of this video that are muted, in order to protect his privacy and the privacy of others.
Interestingly, I used a "suspension suture" to help me to extract this pilar cyst. The walls of a pilar cyst are usually very smooth, reminds me of an olive. I couldn't grasp the cyst with my forceps, so I decided to place a suture into it so I could pull it out.
As you know, pilar cysts are benign growths that occur predominantly on the scalp, and usually have thicker walls than the more common epidermoid cysts. This makes them easier to pop out whole. This bump was very noticeable on his scalp so I think the removal will make him very happy, indeed.

OK, this is a GREAT pilar cyst excision on the scalp of one of my longstanding patients who is so dear to my heart!
She has had this bump on her scalp for many years now, and would love it removed, but wasn't sure what was involved in this process, so she really left it along for many years.
She was happy to learn that it is a pretty simple process, and is just a procedure under local anesthesia. I was quite surprised and excited to see that this pilar cyst just popped out whole with just a little pressure and a squeeze!
I was happy to do this procedure for her, she was so happy to get it removed, and I'm sure all of you will be thrilled to watch this cyst extraction!

It's Throwback Thursday, and on this day I wanted to re-introduce one of my most favorite cyst excisions.
It's one of my favorites for a few reason... 1. He's a really nice guy and I can't believe he walked around with this large cyst on his upper back for so long, so it was really gratifying to remove it for him. 2. The cyst extraction itself was AMAZING, as you can see. 3. His tattoo overlying the cyst presented a little of a challenge. I wanted to remove the epidermoid cyst in it's entirety without compromising his tattoo.
Some viewers have asked why I didn't place my incision line directly next to and parallel to the tattoo lines.. well this would actually distort the line of the tattoo, pulling it towards the incision line and creating a curve in what is supposed to be a straight tattoo line. Therefore, I opted to place the incision in a 90 degree angle to the tattoo, and I made sure to meet up the tattoo line edges when I sutured the edges of the wound closed. When it fully heals, I think it will be tough to see any cyst had ever been there in the first place.

"Raymond" presented with this inflamed neck cyst approximately two weeks prior to this visit. The lesion was obviously infected and needed to be drained but since he was a day away from an overseas trip this was not the right time to get it done.
We have him back today and he's completed the antibiotic prescribed. As we get started I'm thinking that we are going to find a dried epidermoid cyst. You get the idea!

My husband, Dr Rebish, who is also a board certified dermatologist, removed this big cyst on the lower back of one of his patients. Since it had been inflamed before, there is scar tissue in the area, so there are adhesions and fibrous tissue in the area, which then requires that we make a wider excision, because it is not as simple to remove and ensure entire removal when there is scar tissue present or it has been irritated, or contents have been partially removed before.
I think he did a great job , and it looks like it was removed in its entirety so it should likely not reoccur.
