What are Community Partners?
As the hub of the wheel for regional health care delivery, CVMC works closely with other groups based in and around the region to provide a full array of health services to central Vermonters. Some of our community partners include:

Central Vermont Home Health and Hospice
CVHHH (www.cvhhh.org) is a not-for-profit Visiting Nurse Association that belongs to and is responsive to the Central Vermont community. We provide comprehensive, high-quality home health and hospice care to all Central Vermonters, regardless of their illness, diagnosis, geographic remoteness, or ability to pay.In addition, the agency promotes the general welfare of the citizens of Central Vermont with health promotion and long term care services.

Green Mountain Transit Authority (GMTA)
Central Vermont Medical Center recognizes the need for residents in our region to access healthcare services through public transportation within Barre City and Barre Town who were looking to access healthcare services at Barre Health Center through public transportation. GMTA and CVMC have partnered to provide FREE shuttle service to the Barre Health Center and Granite City Primary Care in Barre City. Open to the general public and fare free, this service provides flexible transportation to Barre City and Barre Town areas, including Graniteville, East Barre, South Barre and Websterville. Green Mountain United Way
Green Mountain United Way (www.gmunitedway.org) programs provide services in central Vermont and the Northeast Kingdom for:
- Children and their families
- Elderly
- Physically disabled
- Mentally handicapped
- Homeless
- Victims of violence and abuse
The Green Mountain United Way maintains two office locations in its five-county region of Vermont, the main office being in Berlin, VT and the Northeast Kingdom office in Barton, VT. The Berlin office is located at:

People's Health and Wellness Clinic (PHWC)
The People's Health and Wellness Clinic (www.phwcvt.org) is a free clinic located at 553 N. Main Street in Barre, VT. PHWC provides primary health care and wellness education to uninsured and underinsured central Vermont residents who cannot otherwise afford these services. Dr. Mark Yorra serves as the Clinic’s Medical Director.
PHWC offers medical exams, primary care services, mental health services, holistic health, diet and nutrition, screenings, counseling, referrals, and assistance with prescriptions. To be eligible, patients must have household income below 300% of the poverty level ($32,616 for an individual; $66,456 for a family of four). The Clinic staff also will let you know if you are eligible for any of the public health insurance programs, and other patient financial assistance, and will help you with the applications and through the enrollment process. Washington County Mental Health Services (WCMHS) Inc.
