Today we celebrate the People’s Prince, Prince Jonah Kūhiō Kalaniana`ole on the anniversary of his birth. Prince Kūhiō’s legacy includes his advocacy on behalf of Native Hawaiians, facilitating women’s suffrage (the right to vote) and inspiring the Hawaiian Civic Club movement. Nearly 1 in 3 people in Hawai’i are enrolled in Medicaid, and significant disparities exist for Native Hawaiian and Pacific Islander communities. ACAP Member health plan AlohaCare has been working on innovative efforts to address social determinants and improve health for more than 30 years, including: ➡️ Spearheading programs that incorporate traditional Hawaiian methods of healing and promote culturally competent care ➡️ Partnering with the Hawaii Department of Education to provide financial assistance for enrollees pursuing their GEDs ➡️ Adopting a social referral platform that improves communication with community-based organizations and helps ensure enrollees are connected to needed services. Follow the link below to check out the videos. https://lnkd.in/gc9zq4hZ AlohaCare first convened the Community Health Advisory Partnership (CHAP) in 2022, to help spread the word about upcoming changes to the MedQUEST Medicaid renewal process. Since then, CHAP has grown to include support from 4 Medicaid health plans, the Harry and Jeanette Weinberg Foundation, and the Healthcare Association of Hawaii. CHAP partner organizations include 6 of Hawaiʻi's largest Community Health Centers and over a dozen community based organizations who serve and represent communities with limited English proficiency and others who might be disproportionately impacted by these changes. Now a program of the Hawaii Public Health Institute, CHAP's goal is to work together with trusted community leaders to continue to develop a health communication and promotion model that is collaborative, cross-cultural, and sustainable.
