When clinical psychologist Sarah Mandel was pregnant with her second child, she began preparing for her maternity leave, juggling the demands of her soon-to-be-new baby with the needs of her patients. Noticing a lump in her breast, she assumed it was most likely a clogged milk duct. But a biopsy revealed it was not. When she went into labor, she learned that she had Stage Four cancer—devastating news that forced her to confront terminal illness as she was bringing new life into the world.
But Sarah's illness took a highly improbable turn when, after three months of treatment, her second PET scan showed no evidence of disease. Sarah, however, was unable to celebrate the good news; she was frozen in a dissociated state caused by the emotional whiplash of going from oncology patient to new mother, from a terminal sentence to a shocking reprieve. As a therapist who specialized in trauma work, Sarah had utilized “narrative therapy” to help her patients. Sarah Mandel was a clinical psychologist based in New York City. She lived in Manhattan with her husband and two daughters. She died peacefully on June 1, 2024, weeks shy of her 43rd birthday.

Little Earthquakes is a must read for anyone who struggles with trauma or post traumatic stress. It is an accessible and warm hug of a read. It is magnificently crafted, with precise choice of language and incisive structure. The author has opened up her psychological veins, much as she has opened her body to invasive treatments. The result is a book about hope in the throes of hopelessness. It will make you love Sarah Mandel and all the people who make her life so rich. And it will make you just s little more humble and grateful.
The book is a beacon for patients, families, caregivers, and friends about metastasized cancer. As reviewer Hala Alyan so beautifully shared, I also cried at the end of every early chapter and laughed out loud later on. Most importantly, though, Mandel portrays experiences beyond those that most narratives include and uniquely contextualizes them in the trauma endured. Nevertheless, the reader is not traumatized. The book is full of life.
