The Department of State is now accepting electronic forms for the request of personal records (Request for Individual Access to records Protected under the Privacy Act) and for Third-Party Authorization (Consent for Disclosure of Records Protected under the Privacy Act). The Department of State manages records requests through its Freedom of Information Act, Privacy Act, and the Mandatory Declassification Review programs.
