Partial Hospitalization Program at PrairieCare

The Partial Hospitalization Programs (PHP) for youth and adults provide multi-specialty treatment through group, individual, and family therapy sessions. These intensive treatment options focus on thorough assessment and symptom stabilization while working on healthy living skills. Treatment is guided by a Board Certified Psychiatrist who meets with all patients individually.

  • Monday through Friday
  • 8:45am to 3:40pm
  • Summer Hours: 9:00am to 3:00pm

Length of stay

  • Average length of stay is 3 to 4 weeks

Locations

PHP is a helpful level of care for students who are struggling with symptoms of mental illness that interfere with their ability to function in their daily tasks for childhood or adolescence tasks like; going to school, progressing academically, making and keeping friends, and participating in family life. PHP is also a helpful level of care for adults who may be struggling with symptoms that interfere with their ability to function in their daily lives such as work, caring for themselves, and caring for their families. Potential patients for PHP should seek an Admissions Diagnostic Assessment to help determine the course of mental health treatment that will best meet their needs.

Spirituality Group

Spirituality Group is facilitated by Pastoral Care staff and integrates concepts of spirituality within psychoeducation. This group does not focus on any particular religion or customs, but broadens the topic to invite a variety of views and safe discussions among group members.

 


 

Check-in

Check-in is done every morning for patients in PHP. This is an opportunity to connect with staff and “check in” about the day prior and their morning. Patients can bring up anything of concern, or an accomplishment related to their treatment. We also take this time to monitor basic vital signs such as blood pressure and temperature.

 


 

Medication Management

Medication Management is provided by a licensed prescriber, which may be a Psychiatrist or Clinical Nurse Specialist. These staff members are highly trained and integrate the management of specialized medication to alleviate symptoms and complement the other treatment modalities.

 


 

Process Group

Process Group is offered daily and is run by program therapists. The group process is utilized to help alleviate interpersonal and social dysfunction, to develop communication and relationship skills, to develop identity with allegiance to a group of peers, to increase understanding of illness and coping options, and to foster pro-treatment attitudes and behaviors.

 


 

Recreational Therapy

Recreational Therapy groups are run by a Certified Therapeutic Recreation Specialist (CTRS). RT groups use a variety of activities to educate patients on leisure, life skills, social skills, coping skills, anger management, stress management, conflict resolution/problem-solving, teamwork, and other topics. Recreational Therapy groups may consist of: art projects, music, games, physical activity, skits, worksheets, discussions, or other creative modalities.

 


 

Art Therapy

Art therapy is a unique combination of psychological understanding and visual arts to help clients improve psychological health, cognitive abilities, and sensory-motor functions. The child is given a safe supportive, and structured environment that allows the child to process through their issues and struggles using the art-making process. The art supplies that are available, choose, and utilized by the child help facilitate the therapy process. The positive relationship that develops between therapist and child, provides the ability for the child to address and resolve their issues. Through art therapy, the child will resolve conflicts and problems, develop interpersonal skills, manage behavior, reduce stress, increase self-esteem and self-awareness, and achieve insight.

 


 

Psychoeducation

Psychoeducation is typically facilitated by Licensed Social Workers. This group makes use of various treatment modalities to introduce, discuss and practice the use of coping skills. Some topics include “Reframing”, “Frustration Tolerance”, “Assertive Communication”. This group is experiential, discussion based, or lecture style –depending on the topic at hand for the day.

 


 

Nursing Group

Nursing Group is offered twice a week and run by an RN. Topics are based on healthy living, including nutrition, sleep, hygiene, stress management and more. This group is experiential, discussion based, or lecture style –depending on the topic at hand for the day.

 


 

Social Skills

Social Skills is offered twice a week and led by Psychiatric Technicians. This group allows patients to practice coping skills learned through programming in real life social situations. Patients play games, complete art projects, all while having staff available to coach them through social situations that may cause emotional distress.

 


 

Adult Partial Hospitalization Program in Mankato

The Adult Partial Hospitalization Program (PHP) provides a mix of individual and group mental health psycho-education, empowerment, and treatment in an inclusive and safe environment. Over the course of 4+ weeks, the program enhances individual coping skills and provides a supportive team approach to continue to challenge and foster growth. The PrairieCare team will collaborate with the existing providers to offer potential diagnostic clarification as well as providing medication consultations and adjustments.

 

 

How does PrairieCare collaborate with schools?

PrairieCare values our patient’s academic work, and recognizes the significance of school in the lives of youth. We are proud to host a classroom at each site, which is staffed by the local school district. Classroom staff contact the patient’s school of residence upon intake to begin exchanging schoolwork and clinical information, when approved through a release of information. Educational services at PrairieCare are provided by specially trained staff from District 287 (Brooklyn Park, Edina and Maple Grove), Eastern Carver County Schools (Chaska) ISD 622 (Maplewood), Rochester Public Schools (Rochester) and ISD 77 (Mankato Area Public Schools). While receiving care at PrairieCare, students benefit from customized and engaging curriculum that focus on academic needs while incorporating social and emotional learning. School district staff will manage any changes to educational programming necessary for ongoing success while at PrairieCare. The model includes 2-hours of schooling every day focusing on age appropriate curriculum using engaging multi-dimensional lessons. Licensed teachers use the newest technology such as iPads and smartboards to deliver vibrant and interesting lessons for all ages.

Upon admission, patients are temporarily enrolled in District 287 (Brooklyn Park, Edina and Maple Grove), Eastern Carver County Schools (Chaska) ISD 622 (Maplewood) Rochester Public Schools (Rochester) and ISD 77 (Mankato Area Public Schools). While curriculum and instruction are provided by licensed teachers through the school district, PrairieCare manages the clinical treatment plan. While in PHP each patient’s health information remains protected through HIPAA and educational information is protected through FERPA. Releases of Information (ROI) are requested upon admission so pertinent information can be shared among parties. However, information can only be shared when explicitly authorized by the parent/guardian.

It is commonly expected that patients will require continued and ongoing support after discharge from PHP. The goal of PHP is to help patients acquire the stability necessary for further growth and healing.
