Access care. Find hope and healing today.

Where do you start?

We offer no-cost mental health screenings for all.

Each person experiences mental health and wellness in different ways and we want to offer you the individual care you truly need. PrairieCare’s trained Assessment & Intake Department–or AID Team for short– conducts no-cost mental health screenings over the phone to anyone seeking mental health services.

The brief screening helps to determine the needs of individuals. No need to schedule, just call 952-826-8475 Monday – Friday, between 8:00am and 5:00pm.

In case of a mental health crisis, call 988 or seek the nearest emergency room.

For physical health emergencies, call 911 or seek the nearest emergency room.

Screenings are not full psychiatric evaluations, but are used to determine appropriate next steps to address an individual’s needs. This may include, but is not limited to: outpatient clinic services, intensive outpatient programs, partial hospital, inpatient hospital, residential treatment, or substance abuse treatment.

Testimonials

I cannot express my gratitude enough for this program or my team. I was absolutely hopeless and broken. They changed that in me and changed my family unit. They are so good at their jobs. They have changed my life. “Job” or not, they saw me before I even saw myself. That can’t be taught. That is THEM, who they are.

Dr. Peterson did such an amazing job of asking questions my husband and I never even thought about to try to get to the root of which symptoms were physical/mental and which were possible side effects of medication(s) my son was taking. He took time to listen to everything we had to say, I never felt rushed or dismissed on any of my concerns. We spoke weekly and while he made suggestions, he made sure we knew the decisions were ours to make – giving us complete control on how to proceed with our son’s medications.
