Did you receive a letter that your student has not yet met the immunization requirements and may not be able to start school? |

If your child has received these vaccines, please provide documentation to your child's school via email or traditional mail.
If your child has not yet received these vaccines:
- Please contact your health care provider as soon as possible to schedule an appointment.
- When your student has received these vaccines please submit documentation to your child’s school. This can be done via email or traditional mail.
