About the author
Penny Simkin, PT, is a physical therapist who has specialized in childbirth education and labor support since 1968. She estimates she has prepared over 13,000 women, couples, and siblings for childbirth. She has assisted hundreds of women and couples through childbirth as a doula. She is author or co-author of books for both parents and professionals, including “The Labor Progress Handbook;” “Pregnancy, Childbirth, and the Newborn: The Complete Guide;” “When Survivors Give Birth: Understanding and Healing the Effects of Early Sexual Abuse on Childbearing Women;” “The Birth Partner: A Complete Guide to Childbirth for Dads, Doulas, and All Other Labor Companions,” She has developed teaching materials for birth classes and produced several videos for educators, doulas, and families , the latest of which is for siblings-to-be, “There’s a Baby.” She is co-founder of DONA International (formerly Doulas of North America) and PATTCh (Prevention and Treatment of Traumatic Childbirth). Currently, she serves on the editorial board of the journal, Birth: Issues in Perinatal Care, and serves on the senior faculty of the Simkin Center for Allied Birth Vocations at Bastyr University, which was named in her honor. Survivors might find themselves in the stories in When Survivors Give Birth. But be aware: the content can be VERY triggering for survivors. But chapters like “How survivors can reduce the risk of a traumatic birth” includes useful advice like, “risks can be reduced if the abuse survivor can identify and express her needs or even resolve some troubling emotional issues before the birth,” (emphasis mine 63) so for some the benefit might outweigh the risk. Careful attention is also paid in helping the survivor understand the positive role that a doula can play in her childbirth experience as well as how to get providers to “recognize the needs and fears beneath the surface,” (139).
One of the strongest pieces in WSGB is the section that focuses on the postpartum period. That’s a time when traditionally, the woman gets little or no attention from providers (“come back and see me in 6 weeks,”) but which is a time of great vulnerability and change for even non-survivors. Simkin and Klaus meticulously explore the connection between postpartum mood disorders, for example, and survivors of childhood sexual abuse. Postpartum mood disorder screening for even non-survivors is abysmal but my hope would be that if more providers were to read this book, they might recognize all the more the importance on appropriate screening AND follow-up.
Two things I'd change as a trauma educator and someone who has worked with survivors for 15+ years:
1) Simkin and Klaus don't spend time making the connection between childhood sexual abuse and intimate partner violence. CSA doesn't happen in a vacuum and I wish that was acknowledged more in this book.
2) the book is limited to impact on *child* sexual abuse survivors. Adult survivors of rape will find themselves and their stories absent. This is a huge, but common, miss. The reality is adult survivors can have as many challenges as child survivors.
Also, this book is now almost 20 years old, it is not "trauma-informed" nor inclusive in its language.
When Survivors Give Birth is a terrific reference for anyone who deals with pregnant women in any capacity: from birth doula to ob/gyn. It's a book that providers can return to again and again, both for basics like statistics, symptoms and practical tips on how to deal more sensitively to the needs of survivors of childhood sexual abuse but also for basics such as creative problem solving, active listening and supportive resources.
