In February 2020, President Trump submitted to Congress his FY 2021 budget request for all Federal agencies – the FY 2021 President’s Budget (PB) – including the proposed budget for the National Institutes of Health (NIH) of $38.7 billion.  Visit the links on the Budget Request for more information on the NIH request.

Please note that these materials do not reflect the amendment to the FY 2021 PB request for the National Institute of Allergy and Infectious Diseases (NIAID) released by the Office of Management and Budget in a March 17, 2020 letter to Congress.

National Institutes of Health Funding for Diseases, Conditions, and Research FY 2016 - FY 2021

  • NIH categorizes its spending in a variety of manners to satisfy diverse reporting requirements. Spending is tracked for specific Diseases (Alzheimer’s, Diabetes, Pediatric Cancer, HIV/AIDS, etc.), for various conditions (Infertility, Obesity, etc.) and specific areas of research (Genetics, Mental Health, Nanotechnology, Opioids, etc.). This table provides actual expenditures for FY 2016 through FY 2019, and projected expenditures based on the current year appropriations level and the President’s Budget funding level for FY 2020 and FY 2021, respectively. The projected expenditures are updated each year.

Important Notes:

  • The current year (FY 2020) and budget year (FY 2021) dollar amounts provided represents NIH best estimate on what will be funded for the categories. The figures provided are not allocated to or set aside for these categories.
  • The table does not reconcile mathematically. Funding included in one area may also be included in other areas. For example, Clinical Research includes Clinical Trials and Supporting Activities.

The Office of Budget (OB) advises and supports the Director of the National Institutes of Health (NIH) on budget policy issues affecting the NIH, the medical research community and the public. The OB is responsible for:

  • Devising alternative budget policy scenarios for NIH Leadership consideration and developing associated NIH-wide budget projections in response to continuously evolving budget formulation and execution requirements.
  • Validating and consolidating component organizations’ estimates and justification materials comprising annual budget submissions or special funding requests.
  • Maintaining active and efficient communications with key external stakeholders – such as the Department of Health & Human Services (HHS), the Office of Management & Budget (OMB) and the Congress.
  • Monitoring and coordinating NIH budget activities of the 27 NIH Institutes and Centers (ICs).

The two branches that comprise the OB organization are:

  • Budget Formulation and Modeling Branch
  • Budget Execution and Reporting Branch
