Home health services are provided by LHC Group, Inc.

LHC Group, Inc. provides skilled services for the home-bound patient who requires assistance to return to independence. Our agency is dedicated to providing comprehensive, quality health care, allowing patients to accomplish their individual goals and realiz their full potential. LHC Group, Inc. services are a cost-effective alternative to a lengthy hospital stay and allow for family support and assistance in care for a loved one.

Our office, located adjacent to the campus of Colorado Plains Medical Center at 515 E. Riverview Ave., in Fort Morgan, is open 8:00 a.m. to 5:00 p.m. weekdays, excluding holidays. An on-call RN is available after office hours by phone. Referrals may be made by calling 867-3013. Currently, our geographical area of service includes Morgan County and portions of surrounding counties within a 45-mile radius.

Our Home Health Services are Available 24 Hours a Day

Our nurses provide skilled nursing care in the comfort of your own home and may provide the following:

Wound care - including wound vac
IV therapy
Post-hospitalization assessment and teaching
Education regarding care, treatments, disease process, disease management, and medication

Other services and caretakers may include:

Home Health Aides - are trained in daily activities such as bathing, dressing, personal hygiene and oral care, under the supervision of a registered nurse.

Physical Therapists - evaluate, treat, and instruct patients whose mobility has been compromised. Education in equipment use, exercises, and home safety are provided to the patient and caregivers to help the patient live as independently as possible.

Occupational Therapists - help individuals achieve independence despite disabilities. Evaluation, treatment, and instruction in daily living activities are provided.

Speech Pathologists - provide instruction in skills needed to improve the patient's speech, language, and memory function as well as safe swallowing techniques.

Social Workers - are available to address the social or emotional needs identified by the patient, their family, their physician, and the Home Health Care services team.

Am I Eligible for Home Health Services?

Our services are available to anyone in need of healthcare at home for an acute or chronic medical condition. Home health care is prescribed and directed by the patient's physician. Hospitalization is not required prior to receiving services.

How Will I Pay for Home Health Services?

Our home care services are a cost-effective alternative to a lengthy hospital stay. For those patients who meet criteria, many insurance companies, including Medicare, offer extensive home care benefits to their subscribers. Eligibility and payment programs vary with coverage.

What Do I Do Next?

A referral can be made by your doctor, hospital, nursing home or other patient service agency. An evaluation can be arranged to determine whether home care is appropriate. Screenings can be performed in the hospital, at a long-term care facility, or in your home.
