Key issues:
- Crew man was dragged against the bulwarks and overboard after becoming ensnared in a bight of rope which had caught in a propeller.
- Supervision at the time of the accident was inadequate.
- Communications between the master and the deck were inadequate.
- Company procedures were not in place for a routine but hazardous operation.
- Risk assessments were unsuitable and insufficient.

Accidents (including serious injuries) and hazardous incidents must by law be reported to the MAIB by the quickest possible means. This is so they can be investigated immediately before vital evidence decays, is removed or lost.
