Ideas and theories on how China launched their rocket on accident?

I'm sure most of you have seen the news. I will share a synopsis if you haven't. At some point over the weekend China was doing a static test fire on their new rocket and it accidentally launched. Yes you read that correctly China was doing a static fire of the rocket engine on their new Tainlong-3 rocket when it launched and cleared the pad. Not long after it descended and exploded catastrophically. Obviously safety protocols and other things were not properly in place. I would love to hear from people in the industry nerds enthusiasts and people who have a solid understanding of the potential protocol. How on earth did this happen? I feel like so many safety protocols would have to be ignored or overrided for something like that to happen in NASA or other space programs. I know China cuts corners like crazy but what exactly would have to be ignored or intentionally done to bypass these systems? Please enlighten me, I would love to hear some ideas and learn more about all of this in the process. Cheers and thank you in advance.

I found an Uno in the wild running a penny press machine

So I thought this was kind of neat, the other day I saw one of these penny press machines, which basically let you press a design into a coin. This one was a more modern version though, with a card reader and a hopper filled with pennies, rather than the usual system where you put a coin in to pay and a penny to press.

Anyway, I was kind of curious how it worked, so I took a look around the back, and saw an Uno running everything.

Seems like it takes a signal from the card reader, and then gets the hopper to dispense a penny, and using what I think is a beam break sensor, it can also tell if the penny was successfully dispensed (and then presumably try to dispense another). Just thought this was mildly interesting.

Specifically for those that do data cabling...

How common is it for a client to request for cable certifications?

If the client just requests standard pair testing, do you do just that or do you hold your business/company to a higher standard and certify or at least qualify at the minimum? I do like the idea of qualifying or certifying for every job from a business reputation POV but also we are located in Texas and travel all around the state for jobs so that alone might give piece of mind and time savings by making sure all cables are good to go in order to avoid revisiting.

We do an okay amount of data cabling for commercial to where we can purchase a certifier but definitely isn't necessary and the money could be used elsewhere such as more bonuses or other tools/materials/cables.

Edit: thanks everyone. Looks like I’ll be picking up a couple certifiers!
