I am a pharmacist with ~20 years of community pharmacy experience. I have seen a lot of changes and challenges, many bad, some good. I would like to assist anyone with questions, whether it’ll be about copay, day supplies, why the process to get your medication seems overly excessive, anything! Feel free to reach out to me - your Reddit Pharm.D. (Yes, I did get my doctrine in pharmacy)

Pharmacy work requires its own special laws and regulations and with it comes so many wonderful problems. How many times do you find yourself saying any of these: - Your insurance sets the price - That is your medical card, not your prescription card - Your prescription is expired ... Yes even though you still have refills If so this is the place to be!

0700- Log onto epic, queue is full of morning round orders.

0704- Fix Tylenol order.

0708- Fix morphine order.

0814- 2 patients in ICU now coding, checking orders for stat antibiotics and pressors.

0815- 3 requests for stat docusate. Ignore all.

0820- Phone call for stat docusate. Hang up.

0837- Question new meropenem order. When asked, the interns says that the indication is “I’m not sure but the patient looks sick”.

0850- ID wants to start meropenem. Immediately verify order. Think about how cool and smart ID docs are.

0917- Phone call for stat prn mupirocin. I tube this over as slowly as possible.

0950- Intern asks how to replete mag. I answer with detailed instructions on how to order a mag rider. Intern thanks me.

0953- Fix mag rider order.

1015- NP enters 10 confusing orders. Upon questioning, it turns out that they ordered medications for the wrong patient.

1048- Super stat request for Desitin for a 4 month old.

1100- Lunch break. Some drug company brought us sushi today.

1145- Missing medication call from a nurse. It turns out it was in the nurse’s pocket the whole time.

1150- Fix morphine PCA order.

1201- NP orders kphos rider for a hypokalemic patient with no phos abnormalities. Asks about this order, but NP isn’t sure why they ordered kphos instead of KCl. Asks me what the difference is.

1216- 2 pharmacists and multiple technicians called out again and now it’s just me verifying orders with no one on production for the hour and the queue is going crazy. I get 5 angry calls demanding that I verify stat docusates and Miralax for stable patients. I lie and say I’m working on it.

1240- Queue calms down and admin praises me for my ‘teamwork’ efforts. Even though it’s just me and one tech right now. I go on linkedin and look for openings at other hospitals.

1300- Fix Tylenol order.

1305- Question NP’s TPN order. Why did they take out the levocarnitine today? NP does not know why they did that either. New TPN order with levocarnitine placed.

1307- Renally adjust cefepime. Renally adjust another cefepime. Renally adjust digoxin.

1345- Ketorolac ordered for patient with active GI bleed. Sent Voalte to question order.
