We don't actually get a reliable confirmation of Ned's death in the books either, funnily enough. It's from Arya's pov but she is far away and can't make our the faces, and when Sansa sees his head she says it's so covered in pitch she can't recognise it

I think it's okay if the crit fumble isn't too absurd. "5% chance to literally kill yourself" is a bit ridiculous. But "you lose your grip and have to pick up your weapon again", or "the equipment is damaged, and isn't usable until you patch it up during a long rest", or "you are exposed and the enemy makes a move to take control of the situation" etc

Who are some characters never allowed to move away from their worst moments

I think it comes from only have a superficial understanding of superhero stories. If you just are vaguely aware of Superman, you might think "Superman is all about Truth, Justice, and the American Way, and is just a cheesy indestructible boy scout right? Wouldn't it be fun and cool and interesting to change up that formula a bit?". But if you've actually been reading comics, you'll find that dozens of writers have thought the same thing over several decades, to the point where "evil/flawed superman" is now more of a cliché than "good superman" was.

Other ones include Irredeemable, some versions of Hyperion (particularly in Exiles), New 52 Ultraman, the Sentry (more as a flawed tragic Superman than an evil one) etc

It's something that's often more thought of as campaign design rather than system design. A West Marches campaign can work like this, or anything with a persistent "world", particularly across multiple parties and campaigns.

How did you feel when you encountered the Flood for your first time?

I found it amazing in Halo CE. I had invited one of my friends over to play co-op, and we played through a couple of levels, having no idea that the Flood existed. We got to 343 Guilty Spark and suddenly the atmosphere totally changed, and we were lurking through a swamp with shotguns, and things started coming out of the wall. It was a time when it was still unusual for a game to introduce a twist halfway through, and having an entirely new faction of enemies that weren't even on the box art was a huge surprise.

There's also the comic issue where she's just skipping a rope in a bikini (seemingly naked, as the motion lines cover it up) for the first seven pages. I really don't know how someone could read or watch any She Hulk and think "this is a serious character who would never twerk"

Just starting with non-D&D RPGs - is there a guide to understanding gaming systems & what they're good for?

I'm just going to recommend Dragonbane again. It comes in a nice reasonably priced box with dice and a campaign book. It does emphasise skill rolls, but it's roll-under - you succeed if you roll under your skill on a d20 - and difficulty is all on-the-fly decisions about advantage/disadvantage which are called boons & banes. Unless D&D, you can stack boons/banes - i.e. 3 boons + 1 banes = 2 boons, so you roll 3d20 and take the lowest (remember it's roll under, so lower is better). Players aren't as invincible as in D&D 5e, but it's not a hardcore "almost certain death" game either. There are much fewer special abilities, and so it's much more friendly for homebrewing and making on-the-fly decisions at the table. Combat is a little simpler than D&D 5e as well.

Overall it's got like 80% of what you're describing. For the last part, the issue of players falling back on skill rolls, it is possible to smooth that over at the table - encouraging players to describe what they're trying to do, and if it sounds like it'd just work, to not require a skill roll at all. Similarly, I tend to give players all the information they need, so they don't need insight/perception/search rolls. Dragonbane is the type of game where you could do a "we're basically ignoring perception rolls" homebrew ruling without breaking things.
