If a client is big enough they'll hold literally anything over a small vendor's head to get a discount/free work/get someone fired. When I used to work at marketing agencies I saw bosses fall for it over and over again, to the point that they'd do free work just so they'd be in the good graces of $BIG_CLIENT, because they might bring in the account of their main business in the future (which they never did).

Wouldn't something like that be handled by the PBX in the hospital? Both numbers are registered to the hospital, so with authentication to the telco they could use the same auth keys or whatever token the auth scheme needs.
