I haven’t really seen a lot of POTS new or old. I also haven’t really seen any new SVT. It’s mostly just A-fib that is new. But they’re also old.

On large fires we do rehab. Out in the county one of the volunteer service sets up the rehab area and supplies water, snacks coffee. We check vitals and they have to stay out for at least 20 minutes and drink a bottle of water. In the city the career dept is more involved in the rehab process and release

Try and figure out why they’re in A-fib with RVR. Figure out how long they’ve been in it and then give them fluids.

I mean we carry the antidote for BBs too, just not enough. I would much rather have metoprolol though or have the ability to do cardizem in a drip instead of a bolus.

Should we use diltiazem in the prehospital setting?

It doesn’t matter what they’re on that much. If they’re on a beta blocker already, cardizem might drop their BP more than someone who isn’t. But I don’t think there is actual proof of that in a study.
