Yea my hospital basically has to have a mass casualty event come in to divert anything. We have had 40 patients in our 13 Bed ER with 3 nurses and one doctor and still haven’t diverted.

It’s too hard to prioritize these patients without knowing more info in my opinion. Does the woman just need to use the restroom, or is she experiencing acute urinary retention and her bladder and kidneys are swollen? Does the homeless man just need a bath, or does he have necrotic wounds that led to sepsis? Is this diarhea just from a stomach bug, or is he experiencing severe gastroenteritis and needs IV fluids and antibiotics? It seems like it needs a modifying phrase to give some more information.

At the ER I work at, we hire medics to work our triage. They triage all waiting room patients and assign them to a room and nurse. They can also start IV’s, IO’s, EJ’s, and give medications all the same as a nurse with the added skill of being able to intubate if a physician allows it (usually the doc intubates but sometimes they will just let the medic do it.) It is really a valuable asset to have in a small ER with only one doc around at a time.

I’ve heard of it being abused, but I personally haven’t seen it and my hospital doesn’t treat it as a narc (no witnessed waste needed.)

No, if the child ate half of a CBD gummy as the parent stated, they would be fine. The issue here is the parent lied to poison control, my guess is in an attempt to not get in trouble. It’s like asking someone if you should take your child to the hospital for a cm papercut when it’s really a 10 inch arterial laceration. Poison control are professionals, but they aren’t mind readers.

She told them the child ate half of a gummy, not half of a jar of gummies as they actually did. That’s probably why poison control told her the child would be okay.

I couldn’t imagine someone bringing in a STEMI with an IO unless the pt was actively circling the drain. Someone who is alert and oriented with decent vitals can wait till they get to the ER where there are more resources for an IV or midline or whatever.

Gloves, 2 tourniquets, 2 Abdominal pads (them bois thicc), 4x4 gauze, shears, Kling gauze wrap, and tape should be good enough for any heavy bleeding scenario where you just have to stop the bleeding before the ambulance can arrive. I wouldn’t go fancy with Decompression needles or anything unless you are 100% sure you’d be covered liability wise.

Yea always assess your usuals, in the ER we have many usuals. One of them comes in around 3 times a week for a meal but one time he came in as usual and we discovered he was having a stroke.

We normally give 4 in mine and then move to a different medication so without knowing any better I’d probably think the same.

I’ve seen cases where the person was angry or combative because we took their high away, but it’s not the majority of OD cases. The majority of OD’s that come back combative are due to giving the narcan without properly ventilating the pt so they suddenly jump back into consciousness while being in a severely hypoxic state. This would send anyone into a fight or flight frenzy.
