54,000 women lose their jobs each year in the UK due to pregnancy & maternity discrimination. Despite claims to want to fight "burning injustices", govt response to this problem has been shockingly poor, so perhaps it should be no surprise they treat MPs on mat leave like this.

Pairing is when MPs get matched up with another MP who would vote the opposite way to them. This other MP doesn't vote, so they cancel each other out.

Or, in this case, he does vote and her vote isn’t paired. That should be called out by the Speaker as a breach of process?

It's informal. Not condoning it but there's pretty much SFA the Speaker can do.

Any votes found to have been rigged like this should be made null and void. No one must ever be allowed to get away with gerrymandering the vote. It brings parliament into disrepute
