Register a DPO

Please download and complete the form below if you wish to notify the DPC of the details of your Data Protection Officer (DPO). Data Controllers and Processors are obliged in certain circumstances to appoint a DPO. You may also decide to appoint a DPO on a voluntary basis. Once appointed the details of the DPO must be published and a notification must be sent to notify your supervisory authority.
Also if you need to update the details of your DPO please use the form below to do so.

Breach and Cross Border Breach Notifications

Please complete one of the following forms if you wish to report a breach of personal data to the DPC.

1) If you are a data processor or controller who has engaged in cross border processing that has resulted in or may have resulted in a breach of personal data and you believe this breach is likely to have resulted in a high risk to the rights and freedoms of data subject(s) please download and complete the Cross border Breach Notification form provided.

2) If you are a data processor or controller whose processing has resulted in or may have resulted in a data breach and you believe this breach is likely to have resulted in a high risk to the rights and freedoms of the data subject(s) please download and complete the form below.
Note this breach has not occurred as a direct result from cross border processing.

Raise a Concern with us

Please download and complete the form below if you wish to raise a concern in relation to data protection issues with the DPC. This form allows any individual (data subject) to bring forward an issue with respect to an access request or in scenarios where you feel your data has been breached.
