For medical education- NSFE. This sweet man just lost his wife (and they were inseparable), after a difficult and painful illness. He looks like he has aged 10 years in 10 months. He has had to move into an assisted living facility, and is trying to make new friends, but it's difficult. I usually see him for skin cancer checks, but today I asked him if the bumps on his nose bothered him. He said "YES!". So I tried to remove as many as I could during his visit. As you can see, he really wanted them removed, but doesn't like any discomfort, so I had to be very gentle with him and couldn't get them all. I skip around to different areas in order to minimize the discomfort.. I kind of circle and come back to particularly uncomfortable sites like the nostrils, so as to give him a little break between squeezes. Don't worry, I'll try to get them all! He'll be back in a month! This case, though, is a teaching point for me. No matter how old a patient gets, even if they can't walk well or can't see or hear well... they still are so appreciative if you do something as simple as extract their blackheads.. They can't do it themselves, and it really makes them feel better and look better.

A blackhead is also called an open comedo (single for comedone), and it is a clogged pore in the skin that is open to the air. Keratin (skin protein) and sebum (oil) combine to block the pore. They are often found on the face and trunk. but they can be found anywhere on the body. Blackheads are not clogged with dirt, but it is the exposure to air that causes oxidation turning the internal contents black. They can be extracted using a comedone extractor. He is having a hard time adjusting to his new home and keeping up with all the responsibilities that weigh heavy on his shoulders now. He still has a wonderful sense of humor, and you can see this shine through. I didn't tell you guys that he let me do some more blackhead extractions during our last visit a few weeks ago! Many of you know "Pops" and if you don't follow his story on my YouTube channel! He has many large blackheads and whiteheads, especially around his nose. Here, I try to extract as many as I can, without him getting too uncomfortable. He is darling, and he knows we appreciate him so! Today I noticed that he has a basal cell carcinoma a non life threatening type of skin cancer, which was biopsies during this visit, and will be removed completely in the near future. This has been a year of so many blackhead extractions and cyst excisions!! Many more than all my years as a dermatologist (over 10 years) combined! I didn't realize something that I may do or see on a regular basis as a dermatologist would be so interesting to so many people, and this fascinates me. For many of you, my videos are exciting and you describe them as "gross but oddly satisfying", and for others, you actually watch them to relax you and to help to sleep! Still others have dermatillomania, which is an obsession with and a compulsion to pick your own skin, and these videos help you to diminish this urge. Most importantly, this has hopefully been an educational experience for you, (which is what this all is about, after all). Do you know the difference between a blackhead and a whitehead? Between a comedone and a milium? A pilar cyst versus an epidermoid cyst? An abscess versus a lipoma? If you've watched most of my videos... you BETTER! So I thought to thank you and to also review what we all have learned this last year in dermatology, I would put together a collection of my 10 favorite blackhead extractions I did this year, 2015. Hopefully, these examples will show you how I properly and in a sterile and safe environment, extract blackheads, aka closed comedones. As you know, all of this is done by a board certified dermatologist (me), in the office setting. The intent is to improve and clear the complexion with little to no risk of scarring or other unwanted complications. Hopefully, these videos help you identify issues you may have with your own skin and understand what treatment options you have as well as help you to understand when it's time to see a dermatologist! Download the new Tasty app: http://tstyapp.com/m/mFqUXJtD3E Reserve the One Top: http://bit.ly/2v0iast GALAXY CAKE POP Makes 20 pops INGREDIENTS Cake 1 box cake mix, prepared according to package instructions ⅓ cup vanilla frosting ¼ cup white chocolate, melted Glaze ½ cup water, divided 1 7 grams packet of gelatin ½ cup sugar 2 ounces condensed milk ¾ cup white chocolate Blue food coloring Purple food coloring Green food coloring PREPARATION 1. Preheat oven to 350˚F (180˚C). 2. Prepare the yellow cake batter according to package instructions and pour into greased pan. 3. Bake until a toothpick comes out clean, about 30 minutes. Cool to room temperature. 4. In a medium bowl, crumble the cake. Add vanilla frosting to give some moisture to the cake. If it is too dry add more frosting. 5. Using a small ice cream scoop, portion the cake and shape into a ball. Repeat with remaining cake. 6. Dip a popsicle stick tip into the melted white chocolate and insert into the cake balls. Freeze for at least 30 minutes. 7. In a small bowl, add the ¼ cup water and gelatin and let bloom for 5 minutes. 8. In a medium saucepan, combine ¼ cup water, sugar, and condensed milk, and bring to a boil over high heat. Remove the pan from the heat and add the bloomed gelatin. Whisk until well combined. 9. Add the white chocolate to a bowl. Pour the condensed milk mixture through a sieve into the bowl of chocolate and let it sit for 2 minutes to soften the chocolate. 10. Using a emersion blender, blend the chocolate until smooth. 11.Transfer the mixture into a deep container such as a mug. 12. Squeeze dark blue, purple and green food coloring into the mug or bowl without stirring. 13. Dip the frozen cake pop into the chocolate and twist as you pull up creating a marbled pattern. 14. Stick in a piece of styrofoam and dry the pop completely. Repeat with remaining cake pops. 15. Enjoy!
