Targeting HIV replication
The replication of HIV 1 is a multi-stage process.
Each step is crucial to successful replication and is therefore a potential target of antiretroviral drugs.
Step one is the infection of a suitable host-cell, such as a CD4-positive T-lymphocyte.
Entry of HIV into the cell requires the presence of certain receptors on the cell surface, CD4 -- receptors and co-receptors such as CCR5 or CXCR4.
These receptors interact with protein-complexes, which are embedded in the viral envelope.
These complexes are composed of two glycoproteins:
an extracellular gp 120 and
a transmembrane gp 41
When HIV approaches the target cell gp120 binds to the CD4-receptors. This process is termed attachment.
It promotes further binding to a co-receptor. Co-receptor binding results in a conformational change in gp120.
This allows gp41 to unfold and insert its hydrophobic terminus into the cell membrane.
Gp 41 then folds back on itself.
This draws the virus towards the cell and facilitates the fusion of their membranes.
The viral nucleocapsid enters the host cell and breaks open releasing two viral RNA-strands and 3 essential replication enzymes:
Integrase, Protease and Reverse Transcriptase.
Reverse Transcriptase begins the reverse transcription of viral RNA.
It has two catalytic domains:
The Ribonuclease-H active site
And the polymerase active site
Here single stranded viral RNA is transcribed into an RNA-DNA double helix. Ribonuclease- H breaks down the RNA.
The polymerase then completes the remaining DNA-strand to form a DNA -- double helix.
Now Integrase goes into action.
It cleaves a dinucleotide from each 3-prime end of the DNA creating two sticky ends.
Integrase then transfers the DNA into the cell nucleus and facilitates its integration into the host cell genome.
The host cell genome now contains the genetic information of HIV.
Activation of the cell induces transcription of proviral DNA into messenger RNA.
The viral messenger RNA migrates into the cytoplasm where building blocks for a new virus are synthesised.
Some of them have to be processed by the viral protease.
Protease cleaves longer proteins into smaller core proteins.
This step is crucial to create an infectious virus.
Two viral RNA-strands and the replication enzymes then come together and core proteins assemble around them forming the capsid.
This immature particle leaves the cell acquiring a new envelope of host and viral proteins.
The virus matures and becomes ready to infect other cells.
HIV replicates billions of times per day destroying the hosts` immune cells and eventually causing disease progression.
Drugs which interfere with the key steps of viral replication can stop this fatal process.
Entry into the host cell can be blocked by fusion inhibitors for example.
Inhibition of reverse transcriptase by nucleoside inhibitors or by non-nucleoside Reverse Transcriptase- inhibitors is part of standard antiretroviral regimens.
The action of Integrase can be blocked.
Protease inhibitors are also part of standard antiretroviral therapy.
Each blocked step in viral replication is a step towards better control of HIV disease.

Targeting HIV replication
The replication of HIV 1 is a multi-stage process.
Each step is crucial to successful replication and is therefore a potential target of antiretroviral drugs.
Step one is the infection of a suitable host-cell, such as a CD4-positive T-lymphocyte.
Entry of HIV into the cell requires the presence of certain receptors on the cell surface, CD4 -- receptors and co-receptors such as CCR5 or CXCR4.
These receptors interact with protein-complexes, which are embedded in the viral envelope.
These complexes are composed of two glycoproteins:
an extracellular gp 120 and
a transmembrane gp 41
When HIV approaches the target cell gp120 binds to the CD4-receptors. This process is termed attachment.
It promotes further binding to a co-receptor. Co-receptor binding results in a conformational change in gp120.
This allows gp41 to unfold and insert its hydrophobic terminus into the cell membrane.
Gp 41 then folds back on itself.
This draws the virus towards the cell and facilitates the fusion of their membranes.
The viral nucleocapsid enters the host cell and breaks open releasing two viral RNA-strands and 3 essential replication enzymes:
Integrase, Protease and Reverse Transcriptase.
Reverse Transcriptase begins the reverse transcription of viral RNA.
It has two catalytic domains:
The Ribonuclease-H active site
And the polymerase active site
Here single stranded viral RNA is transcribed into an RNA-DNA double helix. Ribonuclease- H breaks down the RNA.
The polymerase then completes the remaining DNA-strand to form a DNA -- double helix.
Now Integrase goes into action.
It cleaves a dinucleotide from each 3-prime end of the DNA creating two sticky ends.
Integrase then transfers the DNA into the cell nucleus and facilitates its integration into the host cell genome.
The host cell genome now contains the genetic information of HIV.
Activation of the cell induces transcription of proviral DNA into messenger RNA.
The viral messenger RNA migrates into the cytoplasm where building blocks for a new virus are synthesised.
Some of them have to be processed by the viral protease.
Protease cleaves longer proteins into smaller core proteins.
This step is crucial to create an infectious virus.
Two viral RNA-strands and the replication enzymes then come together and core proteins assemble around them forming the capsid.
This immature particle leaves the cell acquiring a new envelope of host and viral proteins.
The virus matures and becomes ready to infect other cells.
HIV replicates billions of times per day destroying the hosts` immune cells and eventually causing disease progression.
Drugs which interfere with the key steps of viral replication can stop this fatal process.

Targeting HIV replication
The replication of HIV 1 is a multi-stage process.
Each step is crucial to successful replication and is therefore a potential target of antiretroviral drugs.
Step one is the infection of a suitable host-cell, such as a CD4-positive T-lymphocyte.
Entry of HIV into the cell requires the presence of certain receptors on the cell surface, CD4 -- receptors and co-receptors such as CCR5 or CXCR4.
These receptors interact with protein-complexes, which are embedded in the viral envelope.
These complexes are composed of two glycoproteins:
an extracellular gp 120 and
a transmembrane gp 41
When HIV approaches the target cell gp120 binds to the CD4-receptors. This process is termed attachment.
It promotes further binding to a co-receptor. Co-receptor binding results in a conformational change in gp120.
This allows gp41 to unfold and insert its hydrophobic terminus into the cell membrane.
Gp 41 then folds back on itself.
This draws the virus towards the cell and facilitates the fusion of their membranes.
The viral nucleocapsid enters the host cell and breaks open releasing two viral RNA-strands and 3 essential replication enzymes:
Integrase, Protease and Reverse Transcriptase.
Reverse Transcriptase begins the reverse transcription of viral RNA.
It has two catalytic domains:
The Ribonuclease-H active site
And the polymerase active site
Here single stranded viral RNA is transcribed into an RNA-DNA double helix. Ribonuclease- H breaks down the RNA.
The polymerase then completes the remaining DNA-strand to form a DNA -- double helix.
Now Integrase goes into action.
It cleaves a dinucleotide from each 3-prime end of the DNA creating two sticky ends.
Integrase then transfers the DNA into the cell nucleus and facilitates its integration into the host cell genome.
The host cell genome now contains the genetic information of HIV.
Activation of the cell induces transcription of proviral DNA into messenger RNA.
The viral messenger RNA migrates into the cytoplasm where building blocks for a new virus are synthesised.
Some of them have to be processed by the viral protease.
Protease cleaves longer proteins into smaller core proteins.
This step is crucial to create an infectious virus.
Two viral RNA-strands and the replication enzymes then come together and core proteins assemble around them forming the capsid.
This immature particle leaves the cell acquiring a new envelope of host and viral proteins.
The virus matures and becomes ready to infect other cells.
HIV replicates billions of times per day destroying the hosts` immune cells and eventually causing disease progression.
Drugs which interfere with the key steps of viral replication can stop this fatal process.

การแปดเปื้อนผลิตภัณฑ์ จากเลือด, การใช้เข็มฉีดยาร่วมกัน เป็นต้น
ส่วนการใช้ของมีคม (เช่นใบมีดโกน ที่ตัดเล็บ) ร่วมกับ ผู้ติดเชื้อ การสัก
การเจาะหู อาจมีโอกาสแปดเปื้อนเลือดที่มีเชื้อได้ แต่จะมีโอกาสติดโรคได้ ก็ต่อ เมื่อมีแผลเปิด และปริมาณเลือด หรือน้ำเหลืองที่เข้าไปในร่างกายมีจำนวนมากพอ
3. การติดต่อจากมารดาที่มีเชื้อสู่ทารก ตั้งแต่ระยะอยู่ในครรภ์ ระยะคลอด
และระยะเลี้ยงดูหลัง คลอด โอกาสที่ทารกจะติดเชื้อจากมารดา ประมาณ

การป้องกันและแก้ปัญหา
ในการป้องกันแก้ไขกับปัญหานี้ ถึงแม้ว่าจะแก้ได้ไม่หมดไปเสียที่เดียวก็ตามแต่ว่าเรา ควรที่จะมีการรณรงค์เกี่ยวกับการป้องกันเอดส์ตามสถานที่ต่างๆ เช่น แหล่งสถานที่บันเทิงต่างๆหรือแม้กระทั่งโรงเรียน และมหาวิทยาลัยโดยป้องกันทุกครั้งเมื่อมีเพศสัมพันธ์ไม่ว่าจะเป็นใครก็ตามเพื่อป้องกันโรคและการรณรงค์ในการสร้างจิตสำนึกการไม่กระทำความรุนแรงต่อสตรีและเด็กเกิดเพราะสตรีและเด็กนั้นเป็นผู้ที่ต้องการให้ผู้ที่เข้มแข็งมาดูแลไม่ใช่ให้ผู้ที่เข้มแข็งมาทำลาย ซึ่งในปัจจุบันนั้นปัญหานี้เป็นปัญหาที่พบอยู่บ่อยครั้ง เช่น การถูกข่มขืนโดยที่ไม่ยินยอมจากฝ่ายหญิงทำให้ผลคือ เกิดโรคอันไม่พึงประสงค์(เอดส์)ตามมาสร้างความเสียหายและทุกข์ทรมารเป็นอย่างมาก

HIV มาจากคำเต็มว่า Human Immunodeficiency Virus ซึ่งหมายถึง เชื้อไวรัสชนิดหนึ่งเมื่อเข้าสู่ร่างกายแล้วจะเข้าไปทำลายภูมิคุ้มกันของมนุษย์
H = Human หมายถึง คน หรือ มนุษย์
I = Immunodeficiency หมายถึง ภูมิต้านทานโรคบกพร่องหรือเสียไป
V = Virus หมายถึง เชื้อโรคที่มีขนาดเล็กมากจนเราไม่สามารถมองเห็นได้ด้วยตาเปล่า แต่ทำให้เกิดโรคร้ายแรงต่างๆได้ ถ้าเข้าไปในร่างกาย
ซึ่งเมื่อHIVเข้าสู่ร่างกาย จะเข้าไปทำลายเม็ดเลือดขาว ทำให้ระบบภูมิคุ้มกันในร่างการเสื่อมสภาพหรือบกพร่องลง เป็นผลทำให้เป็นโรคติดเชื้อ หรือเป็นมะเร็งบางชนิดได้ง่ายกว่าคนปกติ อาการมักจะรุนแรง และเรื้อรังและเสียชิวิตในที่สุด
HIVเป็นไวรัสในกลุ่ม Retrovirus สันนิษฐานว่าเป็นไวรัสที่มีการพัฒนาตัวเองมาจากไวรัสที่ทำให้เกิดโรคเฉพาะในสัตว์เท่านั้นและไม่สามารถ ทำให้เกิดโรคในคนได้ แต่ต่อมามีการพัฒนาขึ้น และค่อยๆ ทำให้เกิดโรคในสัตว์ที่ใกล้เคียงกับคน เช่น ลิง โดยเฉพาะลิงเขียว ในทวีปแอฟริกา (หลังจากนั้นไวรัสเหล่านี้อาจติดมาในคน ในระยะแรกเป็นไวรัสที่ทำให้เกิดมะเร็งของต่อมน้ำเหลืองในคน ต่อมาจึงเกิดเป็นโรคเอดส์ที่เป็นเฉพาะในคนเท่านั้น(Afarican green monkey)
ส่วน AIDS มาจากคำเต็มว่า Acuquired immune Deficiency Syndrome หมายถึงกลุ่มอาการของโรคหรือภูมิคุ้มกันบกพร่องที่เกิดขึ้นภายหลัง
A = Acquired หมายถึง เกิดขึ้นภายหลัง ไม่ได้เป็นมาแต่กำเนิด
I = Immune หมายถึง ระบบภูมิต้านทานหรือระบบภูมิคุ้มกันของร่างกาย
D = Deficiency หมายถึง ความบกพร่อง การขาดไปหรือเสื่อม
S = Syndrome หมายถึง กลุ่มอาการหรือโรคที่มีอาการหลายๆอย่าง

A new drug discovered by scientist that could cure the HIV and Aids. According to the 'Times of Israel', researchers in Israel have identified a protein they claim can reduce the virus in infected patients by 97% in just eight days.
The HIV virus attacks white blood cell(CD4), which is responsible for the body to fight off illnesses. The virus destroys CD4 by making more and more copies of itself. It considers as Aids if a sufferer’s CD4 cells fall below 200 per cubic millimeter of blood.
Scientists at Hebrew University inserted This new drug into test tubes containing the blood of 10 Aids patients and found a positive result. Abraham Loyter (who assist in developing the drug) told Channel 2 in Israel: “With our approach, we are destroying the cells, so there is no chance that the virus will awaken one day. “Because there are no cells, there will be no cells that contain the virus.”

Với cơ chế hoạt động thông thường, virus HIV tấn công CD4, loại tế bào máu trắng có tác dụng giúp cơ thể chống lại bệnh tật như bệnh cảm cúm. Virus HIV tạo ra nhiều bản sao của mình, phá hủy nhanh CD4 hơn. Khi các tế bào CD4 của một người nhiễm HIV giảm xuống dưới 200 mỗi milimét khối máu, chúng sẽ phát triển thành AIDS.
Các nhà khoa học thuộc Đại học Hebrew ở Jerusalem đã đưa loại thuốc mới vào ống nghiệm chứa máu của 10 bệnh nhân nhiễm AIDS.

Lecture Overview:
In this set of lectures, I describe the threat facing the world from the human immunodeficiency virus (HIV) and a bold proposal on how we might meet the challenge of eliminating this disease by engineering the immune system.
In part 1, I provide a broad introduction to viruses, describing their basic properties and my own history of studying the replication of RNA viruses which led to the discovery of reverse transcriptase. I also illustrate the distinguishing features of equilibrium viruses (e.g. the common cold) that have adapted to co-exist with their host and non-equilibrium viruses (e.g. HIV) that have recently jumped from another species, are not adapted to the new host, and which can lead to disastrous outcomes (e.g. loss of immune function with potential lethality in the case of HIV).
In part 2, I describe the growing health problem that is facing the world with the spread of HIV and the limitations of current drug therapies and vaccine strategies. We need new ideas for tackling this problem. Here and in the next segment, I describe bold strategies of using gene therapy to conquer HIV, The approach that I describe in this segment involves gene therapy to produce short hairpin RNAs (siRNA) that target the destruction of a critical co-receptor of HIV, which the viruses that needs to infect cells. I discuss initial proof-of-principle experiments that suggest this approach might be feasible and the next steps needed to develop this idea into a real therapy.
In this last segment, I describe another gene therapy strategy for HIV in which we propose to develop antibody-like proteins that can be expressed by a patient's B cells and will target the HIV virus for destruction. To achieve this objective, hematopoietic (blood) stem cells must to be targeted with the gene, which will ultimately develop into B cells that express the therapeutic molecule. The ultimate goal is to produce a life-long supply of anti-HIV neutralizing antibodies. In this lecture, I describe the molecular methods underlying this strategy and a development path from proof-of-principle studies in mouse to safe trials in humans. This project receives funding from the Bill and Melinda Gates Foundation.

After serving as President of the California Institute of Technology for nine years, in 2006 David Baltimore was appointed President Emeritus and the Robert Andrews Millikan Professor of Biology. Born in New York City, he received his B.A. in Chemistry from Swarthmore College in 1960 and a Ph.D. in 1964 from Rockefeller University, where he returned to serve as President from 1990-91 and faculty member until 1994.
For almost 30 years, Baltimore was a faculty member at Massachusetts Institute of Technology. While his early work was on poliovirus, in 1970 he identified the enzyme reverse transcriptase in tumor virus particles, thus providing strong evidence for a process of RNA to DNA conversion, the existence of which had been hypothesized some years earlier. Baltimore and Howard Temin (with Renato Dulbecco, for related research) shared the 1975 Nobel Prize in Physiology or Medicine for their discovery, which provided the key to understanding the life-cycle of HIV. In the following years, he has contributed widely to the understanding of cancer, AIDS and the molecular basis of the immune response. His present research focuses on control of inflammatory and immune responses as well as on the use of gene therapy methods to treat HIV and cancer in a program called "Engineering Immunity".
Baltimore played an important role in creating a consensus on national science policy regarding recombinant DNA research. He served as founding director of the Whitehead Institute for Biomedical Research at MIT from 1982 until 1990. He co-chaired the 1986 National Academy of Sciences committee on a National Strategy for AIDS and was appointed in 1996 to head the National Institutes of Health AIDS Vaccine Research Committee.
In addition to receiving the Nobel Prize, Baltimore's numerous honors include the 1999 National Medal of Science, election to the National Academy of Sciences in 1974, the Royal Society of London, and the French Academy of Sciences. For 2007/8, he is President of the AAAS. He has published more than 600 peer-reviewed articles.

Human immunodeficiency virus infection and acquired immune deficiency syndrome (HIV/AIDS) is a spectrum of conditions caused by infection with the human immunodeficiency virus (HIV). Following initial infection, a person may experience a brief period of influenza-like illness. This is typically followed by a prolonged period without symptoms. As the infection progresses, it interferes more and more with the immune system, making the person much more susceptible to common infections, like tuberculosis, as well as opportunistic infections and tumors that do not usually affect people who have working immune systems. This portion of the spectrum is often called HIV disease. The late symptoms of the infection are referred to as AIDS. This stage is often complicated by an infection of the lung known as pneumocystis pneumonia, severe weight loss, skin lesions caused by Kaposi's sarcoma, or other AIDS-defining conditions.
HIV is transmitted primarily via unprotected sexual intercourse (including anal and oral sex), contaminated blood transfusions, hypodermic needles, and from mother to child during pregnancy, delivery, or breastfeeding. Some bodily fluids, such as saliva and tears, do not transmit HIV. Common methods of HIV/AIDS prevention include encouraging and practicing safe sex, needle-exchange programs, and treating those who are infected. There is no cure or vaccine; however, antiretroviral treatment can slow the course of the disease and may lead to a near-normal life expectancy. While antiretroviral treatment reduces the risk of death and complications from the disease, these medications are expensive and have side effects. Treatment is recommended as soon as the diagnosis is made. Without treatment, the average survival time after infection with HIV is estimated to be 9 to 11 years, depending on the HIV subtype.
Since its discovery, AIDS has caused an estimated 36 million deaths worldwide (as of 2012). In 2013 it resulted in about 1.34 million deaths. As of 2012, approximately 35.3 million people are living with HIV globally. HIV/AIDS is considered a pandemic—a disease outbreak which is present over a large area and is actively spreading. Genetic research indicates that HIV originated in west-central Africa during the late 19th or early 20th century. AIDS was first recognized by the United States Centers for Disease Control and Prevention (CDC) in 1981 and its cause—HIV infection—was identified in the early part of the decade.
HIV/AIDS has had a great impact on society, both as an illness and as a source of discrimination. The disease also has significant economic impacts. There are many misconceptions about HIV/AIDS such as the belief that it can be transmitted by casual non-sexual contact. The disease has become subject to many controversies involving religion. It has attracted international medical and political attention as well as large-scale funding since it was identified in the 1980s.
