Type 2 diabetes mellitus (T2DM) and obesity are important contributors to non-ischemic heart failure (HF) and atrial fibrillation (AF). There is a 2- to 5-fold increase in HF associated with T2DM, and there is a 5% in HF risk in men and 7% increment in women for every unit increment in BMI, after adjustment for traditional cardiovascular risk factors. Likewise, the risk of AF increases by about 6% per unit increase in BMI. Metabolic cardiomyopathy leads to a number of changes in cardiac structure and function that can be recognized by imaging in the asymptomatic phase, and these parameters can be used for monitoring the progression of disease or the response to therapy. The purpose of this review is to familiarize clinicians with the potential benefits of early detection of preclinical myocardial abnormalities, as well as the mechanisms that might inform interventions to prevent disease progression in patients with T2DM and obesity.

Diastolic dysfunction represents a combination of impaired left ventricular relaxation, restoration forces, myocyte lengthening load, and atrial function culminating in elevated left ventricular filling pressures. Current Doppler echocardiography guidelines recommend the use of E/A to assess diastolic function and E/e' to estimate left ventricular filling pressures. While both parameters have important diagnostic and prognostic implications, their interpretation in the individual patient should be taken in context of a patient's age and the rest of the echocardiogram to describe diastolic function and guide patient management. In this review, we discuss (1) the physiological basis for the E/A and E/e' ratios; (2) the role of these parameters in diagnosing diastolic dysfunction; (3) prognostic implications of abnormalities in E/A and E/e'; (4) special scenarios of the E/A and E/e' ratios that are either useful or challenging when evaluating diastolic function clinically; and (5) the utility of these indices in guiding therapeutic decision-making.

Heart failure (HF) is a chronic syndrome characterized by acute exacerbations. There is significant overlap between respiratory infections and exacerbation of underlying HF. Vaccination against respiratory infections in HF patients could serve as a potential cost-effective intervention to improve patients' quality of life and clinical outcomes. The benefits of influenza vaccination in secondary prevention of ischemic heart disease have been previously studied. However, the evidence for influenza and pneumococcal vaccination specifically in the HF population is less well established. Furthermore, questions around the optimal timing, dose, frequency, and implementation strategies are largely unanswered. This review highlights the current evidence for vaccination against influenza and pneumococcal pneumonia in HF and cardiovascular disease in general. It summarizes current understanding of the pathophysiologic mechanisms in which vaccination may provide cardioprotection. Finally, it offers opportunities for further investigation of the effects of vaccination in the HF population, including basic science, translational research, and large clinical trials.

In each guideline, the decision-making process begins with an assessment of overall cardiovascular risk. Updated treatment thresholds were proposed by each group, and all of them lowered the threshold compared to earlier guidelines. Since release of these new guidelines in 2013 and 2014, additional evidence has emerged to suggest a rationale for extending statin consideration to an even larger proportion of asymptomatic adults - even below 7.5% atherosclerotic cardiovascular disease risk in 10 years. This review discusses new findings since 2013 and proposes strategies emanating from the current guidelines to help clinicians and patients make more informed decisions about long-term statin use, especially pertinent to lower risk individuals.

This interactive session reviews how clinicians are doing in the treatment of high-risk patients and why they might be struggling with prescribing evidence based therapies. The patient’s perspectives and patient related adherence issues or barriers are included also in the discussion followed by the systems of care stumbling blocks. ACC.17 Clinical Focus Session: Next Steps to Optimal Medical Therapy for Heart Failure Patients

Heart failure represents a significant source of morbidity and mortality in the US population and worldwide. It is estimated that the number of persons with heart failure in the U.S. will soon approach 8 million and the toll on life and quality of life will continue to rise. In 2015, the US Food and Drug Agency (FDA) approved two HF medications—the first new pharmacologic therapies for HF in more than a decade. We now have a plethora of drugs and devices appropriate for heart failure and the complexity of care has increased. After two years now of clinical use of the newest agents and coupled with the recent release of updated guidelines, clinicians have questions and need practical guidance on the new pharmacotherapies and how they should be incorporated into individualized treatment regimens with consideration for ethnic/racial and other clinically relevant attributes of patients with HF. Education on the role of guideline-driven MT,(GDMT), including appropriate use of both newer and more familiar medications, is a crucial tool in achieving desired chronic HF outcomes, including reduced hospitalization and readmission rates as well as overall reduced morbidity and mortality.

ACC.17 Clinical Focus Session: Breakthrough Therapies: Your Role in Protecting the Heart of Your Patient with Diabetes

Whether heart disease or diabetes came first, caring for patients with these conditions is complex and places demands on cardiovascular clinicians to stay up-to-date on new findings and pharmaceutical agents and how they may affect or alter care of their patients. This dynamic, interactive session will feature a review of glycemic management and cardiovascular disease, an evaluation of the latest findings of cardiotoxic and cardio-protective profiles of pharmacological diabetes agents and their effectiveness in preventing ASCVD in diabetes. The activity closed with some pragmatic conversations about the cardiovascular clinician’s role and responsibility in caring for the patient with diabetes. With the growing number of T2DM patients in the population, all cardiovascular providers should find this evening session useful and informative.

Insights on Access Barriers and Overcoming Administrative Burdens for New Therapies

Prior authorization and claim denials continue to be some of the top challenges facing physicians. According to a 2012 Kaiser Family Foundation study, U.S. physicians spend more than 868 million hours annually on prior authorization activities—a trend that is only likely to continue.

In October 2016, CardioSurve participants were asked questions related to access and usage of new cardiovascular drug therapies. This research was designed to develop a general understanding from U.S. cardiologists of their perceptions of access to new therapies and usage for their patients with a focus specifically on angiotensin receptor-neprilysin inhibitor (ARNIs), Proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9i) and Novel Oral Anticoagulants (NOACs). The survey was sent to 405 current, active fellows of the American College of Cardiology, and 151 panelists completed the survey for an overall response rate of 37%.
