Jan Tomáš Forman (Czech: [ˈjan ˈtomaːʃ ˈforman]; born 18 February 1932), known as Miloš Forman ([ˈmɪloʃ ˈforman], English /ˈmiːloʊʃ ˈfɔərmən/), is a Czech film director, screenwriter, actor, and professor, who until 1968 lived and worked primarily in the former Czechoslovakia.
Forman was one of the most important directors of the Czechoslovak New Wave. His 1967 film The Fireman's Ball, on the surface a naturalistic representation of an ill-fated social event in a provincial town, was seen by both movie scholars and authorities in Czechoslovakia as a biting satire on Eastern European Communism, resulting in it being banned for many years in Forman's home country.
Since Forman left Czechoslovakia, two of his films, One Flew Over the Cuckoo's Nest and Amadeus, have acquired particular renown, both gaining him an Academy Award for Best Director. One Flew Over the Cuckoo's Nest was the second to win all five major Academy Awards (Best Picture, Actor in Leading Role, Actress in Leading Role, Director, and Screenplay) following It Happened One Night in 1934, an accomplishment not repeated until 1991 by The Silence of the Lambs. Forman was also nominated for a Best Director Oscar for The People vs. Larry Flynt. He has also won Golden Globe, Cannes, Berlinale, BAFTA, Cesar, David di Donatello, European Film Academy, and Czech Lion awards.

There is no doubt that it was Forman’s brilliance as a director that made ‘One Flew Over the Cuckoo’s Nest’ the classic that it remains today. However, ‘One Flew Over the Cuckoo’s Nest’ was only Forman’s second feature film since moving to the United States. You see, Miloš Forman got his start as a director in his home country of Czechoslovakia, with his first big film being 1965’s ‘The Loves of a Blonde’

IGOR: Přátelé, tohle je taková malá radost, vlastně velká... Vážím si možnosti tohle nahrát... Takže s Milošem Formanem TADY A TEĎ, u něj v domě v U.S.A. v Connecticutu. Leden 2013, při natáčení pořadu Pečetě pro Českou televizi... Splnil se mi tím trochu sen, touhle návštěvou, moc si Pana Formana vážím... I to povídání je docela výživné a svérázné, uvidíte... :-) Navezli jsme tam dost techniky, dvě kamery, koleje, světla, dělalo nám dobře, že i Miloš koukal, „jací jsme profíci". Miloš byl navíc v super kondici, a to bylo moc fajn, sršel zdravím i humorem. Amadeus is a 1984 American period drama film directed by Miloš Forman, adapted by Peter Shaffer from his stage play of the same name. The story, set in Vienna, Austria, during the latter half of the 18th century, is a fictionalized biography of Wolfgang Amadeus Mozart. Mozart's music is heard extensively in the soundtrack of the movie. Its central thesis is that Antonio Salieri, an Italian contemporary of Mozart is so driven by jealousy of the latter and his success as a composer that he plans to kill him and to pass off a Requiem, which he secretly commissioned from Mozart as his own, to be premiered at Mozart's funeral.
