AACD Membership Enrollment

Use this form if you have not previously been a member of the AACD. Renewing members, please use the Express Membership Renewal. New membership begins from the date dues are paid and ends the last day of the month the following year. (For example, if you join on August 5, 2017 for 1 year, your membership is paid through August 31, 2018.)

* Additional documentation will be required to maintain member status for all categories with reduced rates.

  • Student Category - please supply a copy of a dated letter of admission into an education program and a photocopy of your current student ID. Please note, this rate is only available to students working towards their dental degree, does not apply to post graduate or other programs.
  • Recent Graduate category - please supply a copy of your diploma. The Recent Graduate member category is available for up to 2 years after completion of dental education.
  • Resident category - please supply a copy of a dated letter from Residency program, this includes all full time post doctorate programs.. The letter should include your residency start and end dates.
  • Full-Time Faculty Category - Please supply a letter of verification from school administrator on official university letterhead stating you are a full-time faculty member.

NOTE: Membership dues are not deductible as a charitable contribution for federal income tax purposes, but should be deductible as a business expense. Check with your tax advisor. Membership dues are non-refundable.
