This is how a small perforation of the eardrum looks like. This is my eardrum after 48 hours of the perforation.
It happened during snorkelling - diving to about 3m without equalizing properly.
Was painful the day it happened. A little painful with water still coming out the next day. Not painful at all on the third day.
There is a mild hearing loss in the higher frequenzies (2,5kHz and up). Will supposedly heal and go back to normal hearing within 3-6 weeks.

How normal eardrum look like ?
This is frequent question ask to me by my patients .
This video show Oto-endoscopy pictures of normal eardrum of both side .
Tympanic membrane is also known as eardrum .
Eardrum receives sound vibrations from the outer air and transmits them to the auditory ossicles , which are tiny bones in the middle ear. It also serves as the lateral wall of the middle ear , separating it from the external auditory canal. The membrane lies across the end of the external canal and looks like a flattened cone with its tip (apex) pointed inward.
The cone of light is in the 5 o’clock position when viewing a normal right ear drum and in the 7 o’clock position for a normal left ear drum. The absence or bent nature of a cone of light can indicate infection.
Eardrum is more or less oval in shape & 9 mm in diameter.It is pearly white in color, thin and semitransparent.The center of the ear drum appears retracted and is known as the umbo. This umbo lies at the apex of the cone of light. Whitish bony extension upwards from the umbo is the handle of the malleus ( Bone in middle ear ).

The client presented with a fully blocked ear due to earwax. They had been using sodium bicarbonate earwax drops but due to the sheer volume of earwax present it was of no benefit. Instead, it caused the earwax to swell and further block the ear.
I performed E-suction® to remove the earwax from the ear canal. There was some remaining earwax stuck on the eardrum, which was being pulled at the same time as when vacuuming the earwax. I therefore instilled some olive oil earwax spray to lubricate and soften the earwax from the eardrum before delicately using a fine end to suction it clear.

Fluid in the middle ear is called Otitis media with effusion (OME). This occurs when there is thick or sticky fluid behind the eardrum in the middle ear behind the ear drum causing deafness.The fluid in OME is often thin and watery. It used to be thought that the longer the fluid was present, the thicker it became. ("Glue ear" is a common name given to OME with thick fluid.). Surgery called myringotomy is performed which drains fluid involves a small incision in the eardrum, so that the fluid can be gently removed and a tube can be inserted.
There are many types of tubes, but all tubes serve the same function. They keep the eardrum open, allow air to enter the middle ear space, and permit fluid in the middle ear to drain. Most tubes will gradually be rejected by the ear and work their way out of the eardrum. As they come out, the eardrum seals behind the tube. Tubes will last four to six months in the eardrum before they come out.
Unlike Grommet, T tubes can generally remain in the ear for prolonged periods of time. They are not commonly rejected.

Eardrum
In human anatomy, the eardrum, or tympanic membrane, is a thin, cone-shaped membrane that separates the external ear from the middle ear in humans and other tetrapods. Its function is to transmit sound from the air to the ossicles inside the middle ear, and then to the oval window in the fluid-filled cochlea. Hence, it ultimately converts and amplifies vibration in air to vibration in fluid. The malleus bone bridges the gap between the eardrum and the other ossicles.
Rupture or perforation of the eardrum can lead to conductive hearing loss. Collapse or retraction of the eardrum can cause conductive hearing loss or cholesteatoma.
Structure
There are two general regions of the tympanic membrane: the pars flaccida in the upper region and the pars tensa. The pars flaccida consists of two layers, is relatively fragile, and is associated with eustachian tube dysfunction and cholesteatomas. The larger pars tensa region consists of three layers: skin, fibrous tissue, and mucosa. It is comparatively robust and is the region most commonly associated with perforations.

A perforated eardrum
This is how a small perforation of the eardrum looks like. This is my eardrum after 48 hours of the perforation. It happened during snorkelling - diving to about 3m without equalizing properly. Was painful the day it happened. A little painful with water still coming out the next day. Not painful at all on the third day. There is a mild hearing loss in the higher frequenzies (2,5kHz and up). Will supposedly heal and go back to normal hearing within 3-6 weeks.

How normal eardrum look like ? This is frequent question ask to me by my patients . This video show Oto-endoscopy pictures of normal eardrum of both side . Tympanic membrane is also known as eardrum . Eardrum receives sound vibrations from the outer air and transmits them to the auditory ossicles , which are tiny bones in the middle ear. It also serves as the lateral wall of the middle ear , separating it from the external auditory canal. The membrane lies across the end of the external canal and looks like a flattened cone with its tip (apex) pointed inward. The cone of light is in the 5 o’clock position when viewing a normal right ear drum and in the 7 o’clock position for a normal left ear drum. Ear Wax Stuck on Eardrum Removed - Mr Neel Raithatha (The Hear Clinic)
The client presented with a fully blocked ear due to earwax. They had been using sodium bicarbonate earwax drops but due to the sheer volume of earwax present it was of no benefit. Instead, it caused the earwax to swell and further block the ear. I performed E-suction® to remove the earwax from the ear canal. There was some remaining earwax stuck on the eardrum, which was being pulled at the same time as when vacuuming the earwax. I therefore instilled some olive oil earwax spray to lubricate and soften the earwax from the eardrum before delicately using a fine end to suction it clear.

It also serves as the lateral wall of the middle ear , separating it from the external auditory canal. The membrane lies across the end of the external canal and looks like a flattened cone with its tip (apex) pointed inward.
The cone of light is in the 5 o’clock position when viewing a normal right ear drum and in the 7 o’clock position for a normal left ear drum. The absence or bent nature of a cone of light can indicate infection.
Eardrum is more or less oval in shape & 9 mm in diameter.It is pearly white in color, thin and semitransparent.The center of the ear drum appears retracted and is known as the umbo. This umbo lies at the apex of the cone of light. Whitish bony extension upwards from the umbo is the handle of the malleus ( Bone in middle ear ).

I performed E-suction® to remove the earwax from the ear canal. There was some remaining earwax stuck on the eardrum, which was being pulled at the same time as when vacuuming the earwax.
