When crews arrived at Capitol Square in Haymarket, they saw large amounts of smoke billowing from the roof of the building.

Initial crews then called for back-up.

Superintendent Greg Rankin from Fire and Rescue NSW said firefighters entered the building and found a fire in an oil vat, which had spread into the building's duct work.

"The fire was initially brought under control by the sprinkler system, then firefighters did the final extinguishment," Mr Rankin said.

"Fire has spread into the duct work directly above the vat, which caused a lot of the smoke."

Around 100 people fled from the building before emergency services arrived.

Three of those were taken to hospital in ambulances to be treated for smoke inhalation.

A NSW Ambulance spokesman said a woman, estimated to be in her 40s, was taken to Sydney Hospital also complaining of chest pain.

A second person in their 40s was taken to Royal Prince Alfred Hospital, while a female patient was taken to St Vincent's Hospital.

The main fire was quickly extinguished, with crews remaining on scene to mop up in the hours after the fire, however the smoke set off several other automatic fire alarms in the CBD.

Mr Rankin said Campbell Street was closed between Pitt Street and George Street as fire trucks responded.

The street was expected to re-open once the alarms were checked and reset.
