Apply to become a member of the Kentucky Dental Association today by downloading a print version Membership Application (PDF) or completing an Interactive Membership Application (PDF) and emailing it to kda@kyda.org.
Complete the membership application and mail it to the KDA offices at 1920 Nelson Miller Parkway, Louisville, KY 40223. Once the application is received, a KDA representative will contact you. If you have any questions, please call us at 502-489-9121.
