Human immunodeficiency virus (HIV) is a lentivirus (a member of the retrovirus family) that causes acquired immunodeficiency syndrome (AIDS), a condition in humans in which progressive failure of the immune system allows life-threatening opportunistic infections and cancers to thrive. Infection with HIV occurs by the transfer of blood, semen, vaginal fluid, pre-ejaculate, or breast milk. Within these bodily fluids, HIV is present as both free virus particles and virus within infected immune cells. The four major routes of transmission are unsafe sex, contaminated needles, breast milk, and transmission from an infected mother to her baby at birth (perinatal transmission). Screening of blood products for HIV has largely eliminated transmission through blood transfusions or infected blood products in the developed world.

In 2008, something incredible happened: a man was cured of HIV. In over 70 million HIV cases, this was a first, and, so far, a last, and we don’t yet understand exactly how he was cured. But if we can cure people of various diseases, like malaria and hepatitis C, why can’t we cure HIV? Janet Iwasa examines the specific traits of the HIV virus that make it so difficult to cure.

Though the drug was highly effective, patients soon began to report bizarre dreams, hallucinations, and feelings of unreality. When South African tabloids started to run stories of efavirenz-motivated rapes and robberies, scientists began to seriously study how efavirenz might produce these unexpected hallucinogenic effects.
Hamilton Morris travels to South Africa to interview efavirenz users and dealers and study how the life-saving medicine became part of a dangerous cocktail called "nyaope."

STORY: KENYA / HIV TREATMENT
TRT: 2.27
SOURCE: UNAIDS
RESTRICTIONS: NONE
LANGUAGE: ENGLISH/ NATS
DATELINE: 7-9 JUNE, 2010 KIBERA, KENYA
1. Pan left, Henry and Monica Ombasa eating
2. SOUNDBITE (English) Henry Ombasa, HIV Patient:
"When I realized when I was tested and I was told I was HIV positive, it really shocked me."
3. Various shots, Henry taking his medicine
4. SOUNDBITE (Swahili) Monica Ombasa, Henry's Wife:
"We discussed with the doctor who counseled us. I realized that even if your husband is HIV positive you can have an HIV negative child."
5. Various shots, Monica with baby
6. Various shots, Henry and Monica playing with baby
7. SOUNDBITE (English) Bernhard Schwartlander, UNAIDS Director:
8. "The science and recent research is very exciting. It shows clearly that with treatment the load of the virus in the blood stream is dramatically reduced and that's the most important factor for the transmission of the virus to other people."
9. Various shots, woman getting tested for HIV
10. Various shots, Michel Sidibé visiting hospital
11. SOUNDBITE (English) Michel Sidibé, UNAIDS Executive Director:
"Funding is so critical because for me today we have been demonstrating with global solidarity that we can produce results. It's not time to scale down but time to scale up."
12. Various shots, drug dispensing station in hospital
13. Various shots, HIV education with young people
14. SOUNDBITE (Swahili) Felistus Munye, 12 Year-old Student:
"We have learnt about HIV and AIDS/, the way we can prevent AIDS. The way AIDS can not be transmitted from one person to another."
15. Various shots, HIV education
STORYLINE:
UNAIDS and its partners are calling for radical reshaping of the AIDS response. This roadmap based on research will be a focal point of the International AIDS conference next week in Vienna.
Henry and Monica Ombasa are the new face of HIV.
SOUNDBITE (English) Henry Ombasa, HIV Patient:
"When I realized when I was tested and I was told I was HIV positive, it really shocked me."
Henry's diagnosis shook his wife who is HIV negative, particularly because she dreamt of having a baby.
SOUNDBITE (Swahili) Monica Ombasa, Henry's Wife:
"We discussed with the doctor who counseled us. I realized that even if your husband is HIV positive you can have an HIV negative child."
Monica's wish came true. Earlier this year, Sheila was born HIV-negative. Since Henry was taking antiretroviral drugs, he was much less infectious.
Treatment is not only giving many families back their future, it is also making a big difference in HIV prevention.
It's also part of what UNAIDS is calling Treatment 2.0. A radically simplified treatment system that UNAIDS Executive Director Michel Sidibé says reduces costs and ensures more people who need treatment can get it during the financial crisis and beyond.
SOUNDBITE (English) Michel Sidibé, UNAIDS Executive Director:
"Funding is so critical because for me today we have been demonstrating with global solidarity that we can produce results. It's not time to scale down but time to scale up."
With simplification every dollar spent on HIV will directly benefit a broader health system.
Treatment 2.0 also means getting affected communities involved, including the next generation of leaders
The number of people accessing HIV treatment has increased 12-fold in just six years. More than 5 million people are on antiretroviral drugs today. But for every two people accessing treatment, there are five new infections.

In 1998, the antiretroviral drug efavirenz was approved for treatment of HIV infection.
