A fail-safe or fail-secure device is one that, in the event of a specific type of failure, responds in a way that will cause no harm, or at least a minimum of harm, to other devices or to personnel.
Fail-safe and fail-secure are similar but distinct concepts. Fail-safe means that a device will not endanger lives or property when it fails. Fail-secure means that access or data will not fall into the wrong hands in a failure. Sometimes the approaches suggest opposite solutions. For example, if a building catches fire, fail-safe systems would unlock doors to ensure quick escape and allow firefighters inside, while fail-secure would lock doors to prevent unauthorized access to the building.
A system's being "fail-safe" means not that failure is impossible or improbable, but rather that the system's design prevents or mitigates unsafe consequences of the system's failure. That is, if and when a "fail-safe" system "fails", it is "safe" or at least no less safe than when it was operating correctly.

This is one of my favorite moments in Sidney Lumet's 1964 Cold War thriller, FAIL-SAFE. It is the confrontation between the hawkish civilian Pentagon advisor, Prof. Groeteschele, and the thoughtful General Black. In the aftermath of the accidental dispatch of a strategic bomber squadron on a run at Moscow, the civilian argues that the U.S. must launch a first strike and attempts to justify it. Black's answer cuts to the matter at hand.

It seemed as if all was lost, when three Fishbeds showed up to save the day. This is an alternate ending to the 1964 film "Fail-Safe", where events take a happier turn than those that ultimately transpired in the movie. P.S. Sorry for the lousy voiceovers.

This is the one role that I'll always remember Dom DeLuise for. It's one of his rare dramatic roles -- a bit part in Sidney Lumet's 1964 cold war thriller, FAIL-SAFE. His anguished take as the Air Force Sergeant (Collins) ordered to give his Soviet counterparts the technical secrets toward defeating our Hustler bombers on their mistaken bomb run was something very special in a film full of great performances. It was exceptional because, for someone not formally trained, he masterfully conveyed the character's conflict with his struggled delivery of the information while in an impossibly tense situation. When his character is done and sits down, you can clearly see the suffering he'll have to live with from that moment forward. What he does with this small role was nothing short of amazing....
