Raw vision: A critical incident investigation has been launched following the death of a youth and the serious injury of another in a collision in Mt Druitt.
Police said officers spotted the boys riding the bike along Carlisle Avenue before they went onto the footpath along Kurrajong Avenue and collided with the side of a ute that was pulling out of a driveway.
They were rushed to The Children's Hospital at Westmead, where Corey, who was driving the bike, died a short time later.
His pillion passenger, Jason, 12, remains in a serious but stable condition with leg and head injuries.
Friends have been posting online tributes to Corey, who according to his Facebook page attended Chifley College Dunheved Campus.
A memorial event for him is due to be held this weekend, and flowers have been left at the scene.
One friend, Tatiana, described Corey as a "a really nice, caring, loving boy".
The friend, who is running a memorial page online, said Corey loved riding bikes and footy.
Other tributes described him as cheeky, smart and bright.
NSW Police Acting Assistant Commissioner Matt Appleton said on Monday that a critical incident investigation would look at "the circumstances of the police engagement with the young boys".
Security camera footage of the incident shows the motorbike with the two boys riding along and seconds later police following.
The footage shows a police car following the bike about 20 seconds behind without its flashing lights or sirens operating.
Witnesses said both boys were looking behind them and not where they were going when the hit the side of the ute. They were not wearing helmets, witnesses said.
The driver of the ute was described as "stunned" and "shaken" and was taken to hospital for mandatory testing but was uninjured.
Police said the investigation is continuing and they have called for any witnesses to come forward.
