Narrator: Like so many expectant parents, Alyson and Dave never planned on a cesarean, or c-section, the surgical method used by obstetricians to birth babies. Alyson: I was scared because it was major surgery, which I didn't want nor did I expect. Narrator: Learning you'll be having a c-section may seem both disappointing and frightening. It's not uncommon for your labor plans to take a sudden turn -- as they have for Alyson and Dave at Bryn Mawr Hospital in Pennsylvania. So it's important to be familiar with your birthing options before the big day. Today, more than one third of all American women deliver their babies by cesarean section. There are a host of reasons for a cesarean delivery, like: Numerous pre-existing medical conditions discovered during your pregnancy that pose a concern for you and your baby. Some examples include high blood pressure, placenta previa, heart disease, and active herpes, among others. You may have had a previous c-section or are choosing to have an elective c-section, which is generally discouraged but becoming more common. Your labor has failed to progress for a variety of reasons -- a condition called distocia -- or you or your baby are in some type of distress. As with any surgery, there are risks to c-sections that you should discuss with your doctor. On the whole, though, a c-section performed by a trained professional is a safe way to deliver a baby. To prepare you for your c-section and help prevent infection, a hospital staff member may shave you. The lower half of your abdomen will be cleaned and swabbed with an antimicrobial solution of iodine. Your surgical team may include up to nine people. Most hospitals allow your partner in the operating room as long as he stays at your side and wears a clean gown and mask. Surgeon: Do you feel that? Narrator: Once it's clear you can't feel pain, the operation will begin. Most c-sections are done through a horizontal skin incision, or bikini cut. In emergency situations or cases of prior surgery, the skin incision may need to be vertical. The vertical or classical uterine incision, historically used in c-sections, is now used only in life-threatening situations, or if your baby is lying sideways. If you have a classical incision, all your future births will have to be c-sections. The low transverse, or Kerr, incision is the most common uterine incision. It's what Alyson will be receiving and is the type that may allow you to attempt a vaginal delivery in your next pregnancy. Two obstetricians move quickly through the first layers of skin, fat, and tissue, cauterizing the incision to prevent excessive bleeding. When they reach the abdominal muscles, they separate them by hand to reveal Alyson's uterus. A final incision is made in the uterus. Birth is moments away. The doctor will guide your baby's head out of the c-section opening. Alyson: I can feel them cutting me and feel them pulling the baby out, but it didn't hurt. Narrator: Your baby's nose and mouth are suctioned, often before being fully delivered. Alyson: I definitely could feel when they actually pulled her out. It was a lot of pressure. Hospital staff: Happy birthday! It's a big girl. That is a big baby. Bigger than I thought! Narrator: Once her umbilical cord is cut and clamped, the anesthesiologist administers an antibiotic through Alyson's IV line to help prevent infection. Not all c-sections are this fast. It takes just three minutes to deliver Alyson's baby. You may have to wait a few minutes to hold your baby because first she'll be taken by a neonatal nurse to get cleaned, weighed, and examined from head to toe. Alyson: It seemed like forever until they actually did show her to me. Alyson in hospital: She's gorgeous! Alyson: Dave did put her right up to my face. For me to touch her cheek to cheek, it was nice. Narrator: Finally Alyson and Dave have their first moments together with their daughter, Zuniyah. The placental sac, which has been Zuniyah's home for 37 weeks, is delivered. Alyson's uterus is then cleaned with surgical cloth. Next a variety of sutures (most of which dissolve) and/or staples may be used to close the incision. This is the longest part of a c-section surgery. It's normal for it to take 30 to 40 minutes to stitch the uterus and the abdomen back together.
