NTSB Vice Chairman Bella Dinh-Zarr's second media briefing on NTSB's launch to Hoboken, NJ train accident, September 30, 2016.
Former investigators say the movie 'Sully' can give the impression that they wanted to smear Captain Chesley Sullenberger, who landed a plane in the Hudson River. They say it could discourage people from cooperating with future investigations. Credit: National Transportation Safety Board (NTSB) The National Transportation Safety Board determines that the probable cause of this accident was the flight crew's mismanagement of the airplane's descent during the visual approach, the pilot flying's unintended deactivation of automatic airspeed control, the flight crew's inadequate monitoring of airspeed, and the flight crew's delayed execution of a go-around after they became aware that the airplane was below acceptable glidepath and airspeed tolerances. Contributing to the accident were; (1) the complexities of the autothrottle and autopilot flight director systems that were inadequately described in Boeing's documentation and Asiana's pilot training, which increased the likelihood of mode error; (2) the flight crew's nonstandard c...

The NTSB is upset over the latest Clint Eastwood movie, Sully. 'Sully' is a biopic that shows the "Miracle on the Hudson", when U.S. Airways flight 1549 landed on the Hudson River and all the passengers survived. The NTSB, which was tasked with investigating the crash, says it's dismayed on how it's portrayed. In the film, the NTSB comes off like angry prosecutors, ready to destroy the career and smear the life of the hero pilot, Chesley "Sully" Sullenberger. The movie shows them looking for any excuse to blame Sullenberger for the mishap. http://hosted2.ap.org/APDEFAULT/4e67281c3f754d0696fbfdee0f3f1469/Article_2016-09-08-US--Sully%20Movie-Investigators/id-ed30a15c3ace41aab3be3158cd676a48 http://www.wochit.com This video was produced by YT Wochit Entertainment using http://wochit.com

Contributing to the accident were the flight crew’s habitual noncompliance with checklists, Gulfstream Aerospace Corporation’s failure to ensure that the G-IV gust lock/throttle lever interlock system would prevent an attempted takeoff with the gust lock engaged, and the Federal Aviation Administration’s failure to d...

Credit: National Transportation Safety Board - NTSB WPR11MA454 Deteriorated Parts Allowed Flutter Which Led to Fatal Crash at 2011 Reno Air Races WASHINGTON - The National Transportation Safety Board determined today that deteriorated locknut inserts found in the highly modified North American P-51D airplane that crashed during the 2011 National Championship Air Races in Reno, Nevada, allowed the trim tab attachment screws to become loose, and even initiated fatigue cracking in one screw. This condition, which resulted in reduced stiffness in the elevator trim system, ultimately led to aerodynamic flutter at racing speed that broke the trim tab linkages, resulting in a loss of controllability and the eventual crash. On September 16, 2011, as the experimental single-seat P-51D airplane "T...

The 1993 Big Bayou Canot train wreck was the derailing of an Amtrak train on the CSXT Big Bayou Canot bridge in northeast Mobile, Alabama, USA, killing 47 and injuring 103, on September 22, 1993. It is the deadliest train wreck in Amtrak's history and the deadliest rail disaster in the United States since the Newark Bay, New Jersey rail accident in 1958. In terms of fatalities and destruction, this wreck substantially exceeded the severity of the Chase, Maryland accident that occurred in 1987. It was caused by a barge hitting the bridge while being pushed by a tow-boat

nadie quiere problemas
y a seguir adelante
por ahí he visto tanta alegría
con la mirada llena y la cabeza vacía
y es así que se nos pasa la vida
pidiéndole al dolor que no nos sangre una herida
nadie sabe... lo que andamos buscando
si un salto hacia la luz
