Pacific Fair boom gate accident: Witness tells of tragic death

A shocked witness has recalled confronting scenes as a 61-year-old woman died in a Gold Coast car park after a freak boom gate accident.

Delivery driver Foti Afantenos was one of the first to rush to the woman's aid as she lay motionless on the ground after the Wednesday afternoon crash.

A witness tells how a 61-year-old woman hit her head on a steel ticket machine at Pacific Fair on the Gold Coast, later dying of her injuries.

It's believed the woman accidentally accelerated after opening her door to try to get her parking ticket out of the recently installed machine at the Pacific Fair shopping centre.

"Instead of putting it on park she pushed down on the accelerator and smacked her head onto the steel ticket machine," Mr Afantenos, 37, said.

"So she's pushed the boom gate. She was still in the car so obviously the door would have slammed back on her.

"She's obviously just fell out of her vehicle door, tumbled onto the ground and that's it, she wasn't moving.

"That's when I ran to her."

Another witness told Mr Afantenos the ticket machine had been causing problems all day.

Mr Afantenos and three or four others felt a faint pulse and light breathing and rolled the lady onto her side, trying to clear her airways, he said.

"They called triple-O, just blood everywhere." That was about 2pm.

But by the time paramedics arrived the delivery driver could no longer feel a pulse.

"Everyone tried to help. Just everyone was in shock, pretty much," he said.

"There was nothing we could do.

"It was just silly, just shouldn't have happened really."

Police say they are still trying to determine how she accidentally hit the accelerator and sustained the fatal injury.

Paid parking was installed at Pacific Fair in June, sparking public anger.

A Pacific Fair spokeswoman told News Corp an investigation was under way.
