Louisiana (i/luːˌiːziˈænə/ or i/ˌluːziˈænə/; French: État de Louisiane, [lwizjan]; Louisiana Creole: Léta de la Lwizyàn) is a state located in the southern region of the United States. Louisiana is the 31st most extensive and the 25th most populous of the 50 United States. Its capital is Baton Rouge and largest city is New Orleans. Louisiana is the only state in the U.S. with political subdivisions termed parishes, which are the local government's equivalent to counties. The largest parish by population is East Baton Rouge Parish, and the largest by land area is Plaquemines. Louisiana is bordered by Arkansas to the north, Mississippi to the east, Texas to the west, and the Gulf of Mexico to the south.
Much of the state's lands were formed from sediment washed down the Mississippi River, leaving enormous deltas and vast areas of coastal marsh and swamp. These contain a rich southern biota; typical examples include birds such as ibis and egrets. There are also many species of tree frogs, and fish such as sturgeon and paddlefish. In more elevated areas, fire is a natural process in the landscape, and has produced extensive areas of longleaf pine forest and wet savannas. These support an exceptionally large number of plant species, including many species of orchids and carnivorous plants.
Barack Hussein Obama II (US i/bəˈrɑːk huːˈseɪn oʊˈbɑːmə/; born August 4, 1961) is an American politician serving as the 44th President of the United States, the first African American to hold the office. Born in Honolulu, Hawaii, Obama is a graduate of Columbia University and Harvard Law School, where he served as president of the Harvard Law Review. He was a community organizer in Chicago before earning his law degree. He worked as a civil rights attorney and taught constitutional law at University of Chicago Law School between 1992 and 2004. He served three terms representing the 13th District in the Illinois Senate from 1997 to 2004, and ran unsuccessfully in the Democratic primary for the United States House of Representatives in 2000 against incumbent Bobby Rush.
In 2004, Obama received national attention during his campaign to represent Illinois in the United States Senate with his victory in the March Democratic Party primary, his keynote address at the Democratic National Convention in July, and his election to the Senate in November. He began his presidential campaign in 2007 and, after a close primary campaign against Hillary Rodham Clinton in 2008, he won sufficient delegates in the Democratic Party primaries to receive the presidential nomination. He then defeated Republican nominee John McCain in the general election, and was inaugurated as president on January 20, 2009. Nine months after his inauguration, Obama was named the 2009 Nobel Peace Prize laureate.
Health care or healthcare is the maintenance or improvement of health via the diagnosis, treatment, and prevention of disease, illness, injury, and other physical and mental impairments in human beings. Health care is delivered by health professionals (providers or practitioners) in allied health professions, chiropractic, physicians, dentistry, midwifery, nursing, medicine, optometry, pharmacy, psychology, and other health professions. It includes the work done in providing primary care, secondary care, and tertiary care, as well as in public health.
Access to health care varies across countries, groups, and individuals, largely influenced by social and economic conditions as well as the health policies in place. Countries and jurisdictions have different policies and plans in relation to the personal and population-based health care goals within their societies. Health care systems are organizations established to meet the health needs of target populations. Their exact configuration varies between national and subnational entities. In some countries and jurisdictions, health care planning is distributed among market participants, whereas in others, planning occurs more centrally among governments or other coordinating bodies. In all cases, according to the World Health Organization (WHO), a well-functioning health care system requires a robust financing mechanism; a well-trained and adequately-paid workforce; reliable information on which to base decisions and policies; and well maintained health facilities and logistics to deliver quality medicines and technologies.
In U.S. politics, a government shutdown is the name for the process the Executive Branch must enter into when the Congress creates a "funding gap" by choosing not to or failing to pass legislation funding government operations and agencies, or, after the Congress passes a bill to fund the government and sends it to the President, the president vetoes that bill. If interim or full-year appropriations are not enacted into law, the United States Constitution and the Antideficiency Act require the federal government begins a "shutdown" of the affected activities. If the funding gap lasts long enough that shutdown plans must be enacted, the law requires the furlough of non-essential personnel and curtailment of agency activities and services. Programs that are funded by laws other than annual appropriations acts (like Social Security) also may be affected by a funding gap, if program execution relies on activities that receive annually appropriated funding. Although the term government shutdown usually refers to what occurs at the federal level, shutdowns have also occurred at the state/territorial and local levels of government.
The Patient Protection and Affordable Care Act (PPACA), commonly called the Affordable Care Act (ACA) or, colloquially, Obamacare, is a United States federal statute signed into law by President Barack Obama on March 23, 2010. Together with the Health Care and Education Reconciliation Act amendment, it represents the most significant regulatory overhaul of the U.S. healthcare system since the passage of Medicare and Medicaid in 1965. Under the act, hospitals and primary physicians would transform their practices financially, technologically and clinically to drive better health outcomes, lower costs and improve their methods of distribution and accessibility.
The ACA was enacted to increase the quality and affordability of health insurance, lower the uninsured rate by expanding public and private insurance coverage, and reduce the costs of healthcare for individuals and the government. It introduced mechanisms like mandates, subsidies, and insurance exchanges. The law requires insurance companies to cover all applicants within new minimum standards and offer the same rates regardless of pre-existing conditions or sex. In 2011, the Congressional Budget Office projected that the ACA would lower both future deficits and Medicare spending.

2013 Louisiana Health Care Conference Part 1 - Louisiana Department of Insurance, Jim Donelon Commissioner of Insurance
INTRODUCTIONS AND WELCOME Donna D. Fraiche, Chair, Louisiana Health Care Commission Baker, Donelson, Bearman, Caldwell & Berkowitz, P.C. The Honorable Jim Donelon, Commissioner of Insurance President, National Association of Insurance Commissioners
