The US now has one of the most expensive health care and insurance systems in the world, while its health "outcomes" - how healthy we are, how long we live, etc. - are mediocre. The reason is the absurd expenses of our peculiar health insurance and care systems because they are organized as private businesses (unlike most other advanced industrial countries in the world). If we had the government buy drugs at huge quantity discounts and pass the savings onto citizens, costs of medicines and medical equipment would drop the far lower levels in other advanced countries; if we rationalized duplication of hospitals, clinics, etc more savings would result; likewise reducing doctor's salaries to those of most other professions and insurance companies to save on their duplications and profits...such obvious steps would allow the US to support its citizens health better and at far less cost than is now the case.

On 2 August 2013, The South African Civil Society Information Service and the Friedrich Ebert Foundation South Africa Office (FES) co-hosted a panel discussion on the theme, "Making the National Health Insurance Scheme (NHI) Work for All South Africans - Can It Be Achieved?"
The South African government's NHI scheme presents an important opportunity to develop a better healthcare system that works for all its citizens. It is a once in a lifetime opportunity to close the gap between public and private healthcare and develop a unified world-class healthcare system that all South Africans could have confidence in -- but how do we achieve this goal?
The questions we put to our panelists were: How does our government intend to develop a unified healthcare system that all South Africans feel is safe and affordable to use? What are the responsibilities of the state, the medical fraternity and the public in general towards developing such a system? Most importantly, how can we use the NHI as an opportunity to build greater social cohesion, in South Africa, around the delivery of an important social good that all South Africans can share in equally?
Prof. David Sanders, Emeritus Professor and founding Director of the School of Public Health at the University of the Western Cape was a keynote speaker at the event. He talked about the challenges South Africans face within the healthcare sector. He reported that 70% of specialists and 50% of doctors are in the private sector serving just 16% of the population.

He believes that the secrecy surrounding the release of the white paper, which has been delayed, is linked to the fact that there might be "a fight going on behind closed doors between people with vested interests who are benefitting from the current inequitable system."

In 1974, the debate had morphed into Nixon vs. Kennedy-Mills vs. Organized labor. Despite the prediction in the second clip shown, the result was stalemate rather than passage in 1974 or 1975.

National health insurance
National health insurance (NHI) – sometimes called statutory health insurance (SHI) – is a legally enforced scheme of health insurance that insures a national population against the costs of health care. It may be administered by the public sector, the private sector, or a combination of both. Funding mechanisms vary with the particular program and country. National or Statutory health insurance does not equate to government-run or government-financed health care, but is usually established by national legislation. In some countries, such as Australia's Medicare system or the UK's National Health Service, contributions to the system are made via general taxation and therefore are not optional even though use of the health scheme it finances is. In practice, of course, most people paying for NHI will join the insurance scheme. Where the NHI scheme involves a choice of multiple insurance funds, the rates of contributions may vary and the person has to choose which insurance fund to belong to.

Health insurance
Health insurance is insurance against the risk of incurring medical expenses among individuals. By estimating the overall risk of health care and health system expenses, among a targeted group, an insurer can develop a routine finance structure, such as a monthly premium or payroll tax, to ensure that money is available to pay for the health care benefits specified in the insurance agreement. The benefit is administered by a central organization such as a government agency, private business, or not-for-profit entity. According to the Health Insurance Association of America, health insurance is defined as "coverage that provides for the payments of benefits as a result of sickness or injury. Includes insurance for losses from accident, medical expense, disability, or accidental death and dismemberment" (pg. 225).

The US now has one of the most expensive health care and insurance systems in the world, while its health "outcomes" - how healthy we are, how long we live, etc. - are mediocre. The reason is the absurd expenses of our peculiar health insurance and care systems because they are organized as private businesses (unlike most other advanced industrial countries in the world). If we had the government buy drugs at huge quantity discounts and pass the savings onto citizens, costs of medicines and medical equipment would drop the far lower levels in other advanced countries; if we rationalized duplication of hospitals, clinics, etc more savings would result; likewise reducing doctor's salaries to those of most other professions and insurance companies to save on their duplications and profits...such obvious steps would allow the US to support its citizens health better and at far less cost than is now the case.

The questions we put to our panelists were: How does our government intend to develop a unified healthcare system that all South Africans feel is safe and affordable to use? What are the responsibilities of the state, the medical fraternity and the public in general towards developing such a system? Most importantly, how can we use the NHI as an opportunity to build greater social cohesion, in South Africa, around the delivery of an important social good that all South Africans can share in equally?
Prof. David Sanders, Emeritus Professor and founding Director of the School of Public Health at the University of the Western Cape was a keynote speaker at the event. The primary public programs are Medicare, a federal social insurance program for seniors and certain disabled individuals; and Medicaid, funded jointly by the federal government and states but administered at the state level, which covers certain very low income children and their families. Together, Medicare and Medicaid accounted for approximately 63 percent of the national inpatient hospital costs in 2011.[55] SCHIP is a federal-state partnership that serves certain children and families who do not qualify for Medicaid but who cannot afford private coverage. Other public programs include military health benefits provided through TRICARE and the Veterans Health Administration and benefits provided through the Indian Health Service. Some states have additional programs for low-income individuals.[56]
In the late 1990s and early 2000s, health advocacy companies began to appear to help patients deal with the complexities of the healthcare system. The complexity of the healthcare system has resulted in a variety of problems for the American public. A study found that 62 percent of persons declaring bankruptcy in 2007 had unpaid medical expenses of $1000 or more, and in 92% of these cases the medical debts exceeded $5000. Nearly 80 percent who filed for bankruptcy had health insurance.[57] The Medicare and Medicaid programs were estimated to soon account for 50 percent of all national health spending.[58] These factors and many others fueled interest in an overhaul of the health care system in the United States. In 2010 President Obama signed into law the Patient Protection and Affordable Care Act. This Act includes an 'individual mandate' that every American must have medical insurance (or pay a fine). Health policy experts such as David Cutler and Jonathan Gruber, as well as the American medical insurance lobby group America's Health Insurance Plans, argued this provision was required in order to provide "guaranteed issue" and a "community rating," which address unpopular features of America's health insurance system such as premium weightings, exclusions for pre-existing conditions, and the pre-screening of insurance applicants. During March 26–28, the Supreme Court heard arguments regarding the validity of the Act. The Patient Protection and Affordable Care Act was determined to be constitutional on June 28, 2012. SCOTUS determined that Congress had the authority to apply the individual mandate within its taxing powers.

Susan Elden - seminar on National Health Insurance in Ghana
In this seminar, Susan Elden presents issues around the implementation of National Health Insurance in Ghana. Although the system in Ghana is not perfect, it has offered a relatively stable health system and it has a lot of potential for the future. As development partners, DFID are particularly interested in the purchasing modalities Ghana has been using to achieve stronger health outcomes. The seminar was delivered at an event titled 'Achieving coverage with quality' that was organised by Mott MacDonald for the Department for International Development (DFID) and London School of Hygiene and Tropical Medicine (LSHTM).
