Associate Professor Rebecca McKetin is a Curtin Senior Research Fellow and Deputy Director at the National Drug Research Institute. Rebecca’s research has focussed on methamphetamine (‘ice’ or ‘crystal meth’), for which she has received an NSW/ACT Young Tall Poppy Science Award. Methamphetamine, also called meth or crystal meth, among other names, is a psychostimulant drug mainly known for its recreational use. Methamphetamine is chemically similar to amphetamine, a drug used to treat attention-deficit hyperactivity disorder, obesity and narcolepsy; but being more potent and highly addictive, methamphetamine is rarely prescribed for medical treatments. Most commonly, the drug is produced illegally, from pseudoephedrine, an ingredient in cold medicines. It can exist as white powder, pills, or bluish-white crystals, and can be consumed by swallowing, smoking, snorting, or injecting.
Methamphetamine acts to increase the amount of a neurotransmitter called dopamine in the brain. Dopamine is at the basis of the brain reward pathway, which is designed to “reward” the body for important behaviors that are essential for survival, such as feeding when hungry. Engaging in enjoyable activities causes dopamine release from dopamine-producing neurons into a space between neurons, where it binds to and stimulates its receptors on the neighboring neuron. This stimulation is believed to produce pleasurable feelings or rewarding effect.
Normally, dopamine molecules are promptly cleared from the synaptic space to ensure that the postsynaptic neurons are not over-stimulated. This is possible thanks to the action of dopamine-transporter, which channels dopamine back to the transmitting neuron.
Methamphetamine binds to dopamine-transporter and blocks dopamine re-uptake. In addition, it can enter the transmitting neuron and trigger more dopamine release. The result is that dopamine builds-up in the synapse to a much greater amount than normal. This produces a continuous over-stimulation of receiving neurons and is responsible for the prolonged and intense euphoria experienced by drug users.
At a low dose, methamphetamine stimulates the brain and can elevate mood and alertness; and by accelerating heart rate and breathing rate, it increases energy in fatigued individuals. It also reduces appetite and promotes weight loss. These seemingly “positive” effects keep users coming back for more, eventually leading to addiction and potential overdose. Long-term drug users may experience extreme weight loss, severe dental damage, and constant hyperactivity which results in anxiety, sleeping disorders and violent behaviors.
Overdose takes the drug’s effects to the extreme and can cause psychosis, heart attacks, seizures, strokes, organ failures, and even death.
Currently, there is no approved pharmacological treatment for methamphetamine addiction; the most effective treatments are cognitive behavioral therapies.

What does a meth user look like? You’re probably not picturing Courtney – and that’s one reason why it’s so important to hear his story.
In the 1990s and early 2000s, meth became the most widely used illicit drug among urban gay men. "Early characterizations of the meth problem in the gay community depicted the party boy or the middle-class white man as the prototype of the meth user," says Dr. Perry Halkitis, professor of applied psychology, global public health, and medicine at New York University.
Public health campaigns in the early 2000s targeted white gay meth users as a way of combating the AIDS epidemic. This, Halkitis believes, created a stigma among middle- and upper-class white gay men and pushed the drug underground in the gay community. And while Halkitis says meth use is still common among all subsets of the gay population, a recent study of his found meth now disproportionately affects HIV-positive and African-American gay men.
To make this short, filmmaker Spencer Macnaughton spent weeks of chatting online with many men and attending a Crystal Meth Anonymous meeting at New York's LGBT Community Center, where 38 gay male ex-meth users shared their recovery stories. Associate Professor Rebecca McKetin is a Curtin Senior Research Fellow and Deputy Director at the National Drug Research Institute. Rebecca’s research has focussed on methamphetamine (‘ice’ or ‘crystal meth’), for which she has received an NSW/ACT Young Tall Poppy Science Award. Her major achievements include developing methods to estimate the number of people dependent on methamphetamine use in Australia, establishing a world-first longitudinal treatment outcomes study for methamphetamine use (the Methamphetamine Treatment Evaluation Study), quantifying the risk of mental health problems associated with the use of the drug, and developing an online intervention for people who use methamphetamine (Breaking the Ice).

Methamphetamine use is a rapidly escalating public health problem in the US. Overdose deaths associated with methamphetamine (mixed with fentanyl) have shown dramatic increases. Clinical challenges in providing treatment to individuals with methamphetamine use disorder, include difficulty in retaining patients in treatment, addressing their psychosis and anhedonia/depression and helping them develop concrete behavioral plans. At present, there are no FDA-approved medications for the treatment of individuals who are dependent upon methamphetamine, although several have some (including mirtazapine, bupropion and naltrexone) have some evidence of usefulness. The current approach with the strongest empirical support are contingency management (motivational incentives) and Community Reinforcement Approach. Other behavioral strategies (including physical exercise, CBT and transcranial brain stimulation) have supportive evidence.
Richard A. Rawson, Ph.D., is Professor Emeritus at the UCLA Department of Psychiatry and a Research Professor at the Vermont Center for Behavior and Health at the University of Vermont. He received a Ph.D. in experimental psychology from the University of Vermont in 1974. Dr. Rawson conducted numerous clinical trials on pharmacological and psychosocial/behavioral addiction treatments for the treatment of individuals with cocaine and methamphetamine disorders. He was a member of the Federal Methamphetamine Advisory Group to Attorney General Janet Reno (1996-2000) and represented the US at numerous international meetings on methamphetamine. He has led addiction research and training projects for the United Nations, the World Health Organization and the U.S. State Department, exporting science-based knowledge to many parts of the world. Dr. Rawson has published 3 books, 40 book chapters, and over 250 peer-reviewed articles and has conducted over 1,000 workshops, paper presentations, and training sessions.
