In this documentary,
Dr. Robert Kerstein and
Dr. Nick Yiannios discuss the usefulness of
DTR (Disclusion
Time Reduction) therapy for
TMD patients. Designed for anyone who wants to learn about this highly successful, hi-tech "
TMJ" procedure. Topics addressed are:
1) Why splints are not needed for TMJ patients 85% of the time
2) How friction in the dynamic bite causes TMJ pain & how DTR can eliminate this pain most of the time
3) Why muscular TMD issues cause 85% of the pain that TMJ patients experience, and how DTR can help most all of that 85%
4) Why it's critical to ascertain the status of both of the patients TMJ's-objectively, with digital metrics
5) How many visits it takes to accomplish DTR
6) How long it takes a patient to respond to the DTR therapy
7) How to screen the patient to know if they are a DTR candidate
8) The role of the lateral pterygoid muscles in the TMJ patient
9) Why lactic acid is important for TMD patients
10) How the TM disc can become damaged
11) Splints and Centric
Relation, or not
12) Ear issues & DTR (tinnitus vs. sharp pain)
13)
Headache resolution after DTR
14)
Cure TMD with DTR?
15) The ICAGD Occlusal (bite) Adjustment defined & described
16) Is DTR therapy painful?
17) Is the bite related to TMD?
18) Do you need local anesthetic for DTR?
19)
Insurance coverage for DTR?
20) Where can you have DTR done?
21) Why are so few doctors knowledgable about DTR?
22) 30 years of scientific research validating DTR
23) The T-Scan and its usefulness in dental medicine
24) The gross inaccuracy of bite ribbon
25) Usefulness of
EMG for DTR
26) Is DTR lasting?
27)
Neck & shoulder spasms-will DTR help with this?
28) Can DTR stop clenching and grinding?
29) What you can expect from DTR therapy
30)
Muscular TMD symptoms & DTR applicability
31) What does DTR do?
32)
Bite/occlusal adjustments explained
33) References to the scientific literature included
34) A reference to the new,
1st edition textbook on digital dental occlusion that will be released by
IGI Global in January of
2015. Dr. Kerstein is the editor of that text. Dr. Yiannios is a contributing author to that textbook. Dr. Kerstein and Dr. Yiannios are both highly experienced in the usage of DTR TMD therapy, and are quite possibly the two most experienced DTR clinicans in the world.
NOTE: though more than 15% of the population has damage in their TMJ's, ~15% have issues that are maladapted and unstable and not amenable to Occlusal Adjustment treatment such as DTR. Therefore, more than 85% of "TMJ" patients have joint damage (approximately 50-60% actually), but ~15% of the 50-60% are serious enough to be considered unstable and maladapted, truly damaged joints that can or will cause painful symptomatology.
Again, as stated in the documentary, damage to the medial pole of the TM cartilagenous meniscus which does not reduce back on the mandibular condylar head is the threshold where DTR therapy becomes less predictable for a patient. To learn more do one or more of the following:
