Mouen Khashab, MD, Assistant Professor of Medicine and Director of Therapeutic Endoscopy, Johns Hopkins Gastroenterology & Hepatology, discusses Peroral Endoscopic Myotomy (POEM) - how it is performed, who is eligible for the procedure, expected outcomes and the key benefits. For more information, visit the Division of Gastroenterology & Hepatology webpage at http://www.hopkinsmedicine.org/gastroenterology_hepatology/clinical_services/advanced_endoscopy/peroral_endoscopic_myotomy.html Or call 443-287-1960. Questions answered: 1. What is POEM? (0:11) 2. What is Esophageal Achalasia? (0:30) 3. Who does this condition effect and what symptoms are common? (1:20) 4. How is POEM performed? (1:46) 5. Who is eligible for the procedure? (3:12) 6. What are the expected outcomes? (3:36) 7. What ar...
Video shows a surgery of a 55 years old woman complaining of progressive disphagia for 2 years, before the surgery she only tolarate ingest of liquid diet. Seriography showed large diameter of the esophagus, manometry showed hiperconstriction of the inferior esophagus sphincter, that confirmed Idiopatic Achalasia. Laparoscopic Heller Myotomy + Do'r Fundoplication was indicated and performed. Ultracision and Monopolar Hook were used in the dissection. During the surgery, an acessory hepatic artery was preserved in the lesser omentum. The dissection of anterior vagus trunk was showed (that avoids vagus injury during the myotomy and during wrap confection). The wrap was done with continuous suture of prolene 2.0. At the end of the surgery no drain left into the cavity. Pacient started oral wat...
The PerOral Endoscopic Myotomy (POEM) is a novel endoluminal technique that allows cutting of the outer muscular layers of the esophagus without surgical intervention. For more information, please call 507-284-2808
A surgical procedure called the Heller myotomy has become the treatment of choice and extensively practiced by laparoscopic surgeons. This video demonstrate Laparoscopic Heller Myotomy for Achalasia (Swallowing Disorder)
SAGES 2012 Bonus Video - Megan M Gilmore, MD, Shanu N Kothari, MD
This video demonstrates the surgical approach for the treatment of achalasia. This is considered the "gold standard" as it is the most durable and safest approach with near 90% long term success compared to 60% or less with endoscopic approaches.
The surgical treatment for patients with Achalasia is the Heller Myotomy by laparoscopic approach. This procedure solves the issue of the lack of relaxation at the lower esophageal sphincter during deglutition, however up to 20% of patients experience gastroesophageal reflux. This is a trailer video, watch the full version free here: http://aischannel.com/society/laparoscopic-heller-myotomy/
Dr. Raja explains in this whiteboard video the treatment of achalasia, from medical management to surgery, in this whiteboard video. Surgery options include Heller Myotomy and the minimally invasive Peroral endoscopic myotomy (POEM) surgery.
Teaching video for surgeons Jonathan Carter, MD UCSF Department of Surgery
Video shows a edited surgery for achalasia called Heller Myotomy + Dor Fundoplication. The surgery consists in the cutting of the lower esophageal sphincter to allow the food to pass from the esophagus to the stomach (in people who have a disease that causes this sphincter to be contracted all the time) and a gastric wrap to prevent reflux. The key steps are demonstrated and all the landmarks ar showed.

Mr. Trouble never hangs around
When he hears this mighty sound...
"Here I come to save the day!"
That means that Mighty Mouse is on the way.
Yessir when there is a wrong to right
Mighty Mouse will join the fight
On the sea or on the land
He gets the situation well in hand
So though we are in danger
We never despair
Cause we know that where there's danger
He is there!
He is there!
On the land!
On the sea!
In the air!
We're not worryin' at all
We're just listenin' for his call
"Here I come to save the day!"
