This is the first of three lecturettes on the topic of central serous chorioretinopathy (CSC). The intended audience includes ophthalmology residents or anyone who would like to brush up on CSC. This lecture is not primarily based on the AAO's BCSC series, but rather it draws primarily from the ophthalmic peer-reviewed literature on CSC. As such, it is more clinically-oriented than it is towards mnemonic or test-taking pearls. It may also be useful to the layperson. Please let me know if you see any errors that I should address. Central serous retinopathy (CSR), also known as central serous chorioretinopathy (CSC), is an eye disease which causes visual impairment, often temporary, usually in one eye. When the disorder is active it is characterized by leakage of fluid under the retina that has a propensity to accumulate under the central macula. This results in blurred or distorted vision (metamorphopsia). A blurred or gray spot in the central visual field is common when the retina is detached. Reduced visual acuity may persist after the fluid has disappeared.
The disease is considered idiopathic but mostly affects white males in the age group 20 to 50 and occasionally other groups. The condition is believed to be exacerbated by stress or corticosteroid use.

In physiology, the term serous fluid or serosal fluid is used for various bodily fluids that are typically pale yellow and transparent, and of a benign nature, that fill the inside of body cavities. Serous fluid originates from serous glands, with secretions enriched with proteins and water. Serous fluid may also originate from mixed glands, which contain both mucous and serous cells. A common trait of serous fluids is their role in assisting digestion, excretion, and respiration.
In medical fields, especially cytopathology, serous fluid is a synonym for effusion fluids from various body cavities. There are many causes of effusions which include involvement of the cavity by cancer. Cytopathology evaluation is recommended to evaluate the causes of effusions in these cavities.

Objective: To illustrate a case demonstration of laparoscopic modified radical hysterectomy in a patient with uterine papillary serous carcinoma involving the endocervical canal.
Methods: This 47 year-old, para-3 woman with no family history of cancer and 3 prior cesarean deliveries presented with postmenopausal bleeding. Endometrial biopsy revealed a mixed poorly differentiated papillary serous and endometrioid carcinoma. On initial examination, a cervical lesion was noted and endocervical curettage was positive for a poorly differentiated adenocarcinoma with squamoid features. Due to the endocervical involvement, modified radical hysterectomy was recommended. All options for management were reviewed with the patient, and she consented to laparoscopic surgical staging.
Results: She underwent laparoscopic modified radical hysterectomy, bilateral salpingo-oophorectomy, bilateral pelvic and paraaortic lymphadenectomy, and omentectomy without complication. Her total operative time was 330 minutes with a total blood loss of 100cc. She was discharged on postoperative day 3 and had postoperative urinary retention that was treated conservatively until she could void appropriately on postoperative day 14. Final pathology revealed a stage IIIC uterine papillary serous carcinoma. She underwent postoperative chemotherapy with Taxol, Adriamycin, and Cisplatin. She is currently with no evidence of disease at 10 months follow-up.
Conclusions: Laparoscopic modified radical hysterectomy with staging as described is a feasible treatment approach and alternative to laparotomy for patients with advanced uterine papillary serous carcinoma.
