This is a patient of Dr Jeff Rebish, who is a board certified dermatologist, my colleague, and my husband. :)
Dr Rebish is usually the one who does the nail biopsies. Well, actually, not usually, but ALWAYS. He's the only one in our office who does nail biopsies, because I don't do them. I don't like them, I don't like doing them, so thank goodness I can send any patient of mine who has an issue like this to him! In exchange, he sends me anything I can pop out of the skin and that's just fine with me!
This is a patient of his who has a growth under his nail, in his nail bed. This growth is trapped under his nail, but is growing and is causing pain. There are two ways to biopsy a growth under the nail. One way is to do a punch biopsy, essentially taking a core of nail and the tissue underneath. The other is to remove the entire nail, which is what Dr Rebish opted to do in this case, because he couldn't evaluate what the lesion looked like growing underneath. First, of course, Dr Rebish did a digital nerve block, essentially numbing up the entire area so he can remove the nail without any pain or discomfort.
When he removed the nail, the mass growing underneath, was soft and friable. He thinks it may be a glomus tumor (a benign growth), but it could be malignant. We will find out the biopsy results very soon, and I'll add the result to this description when I receive it.

Nail (anatomy)
A nail is a horn-like envelope covering the tips of the fingers and toes in humans, most non-human primates, and a few other mammals. Nails are similar to claws in other animals. Fingernails and toenails are made of a tough protective protein called keratin. This protein is also found in the hooves and horns of different animals. It is made up of dead skin cells.
Structure
The nail consists of the nail plate, the nail matrix and the nail bed below it, and the grooves surrounding it.
Parts of the nail
The matrix, sometimes called the matrix unguis, keratogenous membrane, nail matrix, or onychostroma, is the tissue (or germinal matrix) which the nail protects. It is the part of the nail bed that is beneath the nail and contains nerves, lymph and blood vessels. The matrix is responsible for producing cells that become the nail plate. The width and thickness of the nail plate is determined by the size, length, and thickness of the matrix, while the shape of the fingertip itself shows if the nail plate is flat, arched or hooked. The matrix will continue to grow as long as it receives nutrition and remains in a healthy condition. As new nail plate cells are made, they push older nail plate cells forward; and in this way older cells become compressed, flat, and translucent. This makes the capillaries in the nail bed below visible, resulting in a pink color.

Dr Rebish is usually the one who does the nail biopsies. Well, actually, not usually, but ALWAYS. He's the only one in our office who does nail biopsies, because I don't do them. I don't like them, I don't like doing them, so thank goodness I can send any patient of mine who has an issue like this to him! In exchange, he sends me anything I can pop out of the skin and that's just fine with me!
This is a patient of his who has a growth under his nail, in his nail bed. This growth is trapped under his nail, but is growing and is causing pain. There are two ways to biopsy a growth under the nail. One way is to do a punch biopsy, essentially taking a core of nail and the tissue underneath. The other is to remove the entire nail, which is what Dr Rebish opted to do in this case, because he couldn't evaluate what the lesion looked like growing underneath. First, of course, Dr Rebish did a digital nerve block, essentially numbing up the entire area so he can remove the nail without any pain or discomfort.
When he removed the nail, the mass growing underneath, was soft and friable. He thinks it may be a glomus tumor (a benign growth), but it could be malignant. We will find out the biopsy results very soon, and I'll add the result to this description when I receive it.

The United States has expelled two Russian officials following an attack on an American diplomat outside the US Embassy in Moscow last month, the State Department says. The US diplomat was attacked by a Russian guard on June 6 and the incident was caught on a security camera ... "On June 17, we expelled two Russian officials from the United States to respond to this attack," Kirby added....
