    In this mini lecture, Dr. Gish examines the Hepatitis B and how the Hepatitis B Virus can affect your liver and your health. The term "hepatitis" means inflammation of the liver. When the liver is inflamed, it can have a harder time doing some of its jobs. Hepatitis B is caused by the hepatitis B virus. Many people who get hepatitis B as adults can get rid of the virus on their own. But others, especially children and some adults, can develop chronic (or lifelong) hepatitis B that can lead to liver damage, and sometimes the liver damage can then lead to liver cancer and death.

Early on, "IgM-type" antibodies to the core antigen called IgM-anti-HBcAg antibodies are detectable (i.e. +) early in infection. ------------------------------------------------------- CHRONIC INFECTION: anti-HBsAg antibodies are formed. The IgM anti-HBcAg formed during the earlier acute infection become "IgG-type" anti-HBcAg antibodies by this time. This is because levels of "IgM" type antibodies wane and undergo isotype switching to "IgG type" anti-HBcAg antibodies. Anti-HBcAg is an antibody marker of infection. -------------------------------------------------------------------- Individuals who received the HBV vaccine mount a measurable anti-HBsAg antibody response in the blood, so if the vaccinated individual becomes exposed to the virus, anti-HbsAg can bind to HBsAg on the viral envelope. This renders the virus non-infective, and as a result, appropriately vaccinated persons will not develop signs of infection after exposure (i.e. they will have undetectable HBsAg, HBcAg, HBeAg, HBV DNA, anti-HBcAg after an exposure). Again, vaccinated individuals will have protective anti-HBsAg (antibodies that can bind to the HBV surface envelope protein). ------------------------------------------------------- (1) HBeAg and HBV DNA are the infectious particles of the virus. (2) HBsAg + for greater than 6 months = chronic HBV infection. --------------------------------------------------------------------------------- SUMMARY OF INFECTIOUS MARKERS IN UNVACCINATED PERSON: (a) HBsAg is the 1st marker to appear in the blood of unvaccinated person (followed by HBeAg & HBV DNA). These appear and disappear over months. (b) Anti-HBc-IgM + around 2 months, which slowly switches to anti-HBc-IgG, which persists. (c) antibodies to HBsAg (anti-HbsAg) reappear between 5-6 months along with anti-HBe* antibodies. *(e= e antigen) ---------------------------------------------------- Use caution when interpreting lab tests -- false positives and negatives sometimes occur, so always talk to your provider when interpreting tests. *The animation developer has no relevant financial interests*

Perinatal infection is a major route of infection in endemic (mainly developing) countries.[5] Other risk factors for developing HBV infection include working in a healthcare setting, transfusions, and dialysis, acupuncture, tattooing, extended overseas travel and residence in an institution.However, Hepatitis B viruses cannot be spread by holding hands, sharing eating utensils or drinking glasses, kissing, hugging, coughing, sneezing, or breastfeeding. The acute illness causes liver inflammation, vomiting, jaundice and, rarely, death. Chronic hepatitis B may eventually cause cirrhosis and liver cancer—a disease with poor response to all but a few current therapies. The infection is preventable by vaccination. Hepatitis B virus is an hepadnavirus—hepa from hepatotropic (attracted to the liver) and dna because it is a DNA virus[12]—and it has a circular genome of partially double-stranded DNA.
