An ovarian cyst is usually a fluid filled sac that is found within an ovary. An ovarian follicle which is larger that is two centimeters in size is termed as an ovarian cyst. There is a buildup of blood and color changes as time goes by turning brown. Progesterone functions with estrogen by promoting menstrual cycle changes in the endometrium.

Pathophysiology Underlying cause is unkown but there is a much higher LH production than FSH production. LH activates theca cells to produce testosterone and the granulosa cells are usually secreted by FSH and there will be low estrogen. In obese patients there is a lot of aromatase which converts testosterone into estrogen. This can lead to endometrial hyperplasia. There is also insulin resistance due to post-receptor problem, not necessarily associated with weight. When this occurs the ovarian cyst may contain developmentally mature skin that is inclusive of hair follicles, sweat glands and in cases even clumps of hair; pouches of cartilage, teeth, thyroid tissue, fat, blood, bone, nails and eyes may also be found in this type of ovarian cyst. More often than not a dermoid cyst will appear to be benign rather than malignant.
*Endometiroma is also known as an endometrioid cyst, endometrial cyst or a chocolate cyst. This type of ovarian cyst is caused by a condition called endometriosis where a small patch of endometrial tissue, which is the mucous membrane that the uterine wall is composed of; bleeds is sloughed off, is transplanted and grows inside the ovaries. There is a buildup of blood and color changes as time goes by turning brown. Once the cyst ruptures the contents of the cyst is spread into the pelvis and surface of the bladder, bowel, uterus and other related spaces in the area. Treatment for this type of ovarian cyst may be medical with the use of non-steroidal anti inflammatory drugs. Most commonly endometrial cysts may also be treated via surgery. In younger patients conservative surgery may be opted for to protect their fertility. Definitive surgery may also be used.
*Cystedenomas are abnormal ovarian cysts that develop from ovarian tissue and may be enlarged with a watery liquid or gel like, mucous material. They may grow up to the size of 12 inches or more and may cause a twisting of the ovary.
*Polycystic ovarian disease or polycystic ovarian syndrome presents itself with thickened sclerotic capsules and an abnormally large amount of follicles. The ovary may secrete higher levels of testosterone and estrogen in this case. In this case the ovary appears to be larger in size with numerous growths around it. This type of ovarian cyst is pathological and associated with tumors.

Generally speaking, women are at higher risk for ectopic pregnancy if they've had surgery, if they have congenital anomalies or infection that affected the fallopian tubes and their anatomy. Basically, what an ectopic pregnancy is is a pregnancy that starts to grow outside of the uterus. So it could be in the fallopian tube, it could be in other parts of the abdomen as well, but basically, if there's a blockage, or scar tissue, or something affecting the fallopian tube, there is a chance that it could happen there. So again, your doctor would be able to give you more information about this, based on their knowledge of your situation.
Generally speaking, women are at a higher risk for ectopic pregnancy if they've had pelvic infection, surgery, or if they were born with congenital anomalies that affects the normal anatomy of a fallopian tube. So because you had infection and surgery, that does put you at higher risk, generally speaking, but again, your doctor will be able to give you the best information about it, based on their knowledge of your circumstances. If the doctor has checked out the left fallopian tube and everything is patent and looks good, then you should be good to go. This video will cover Polycystic Ovary Syndrome (PCOS) for USMLE. We will be going over pathophysiology, clinical signs and symptoms and management.
Pathophysiology
Underlying cause is unkown but there is a much higher LH production than FSH production. LH activates theca cells to produce testosterone and the granulosa cells are usually secreted by FSH and there will be low estrogen. In obese patients there is a lot of aromatase which converts testosterone into estrogen. This can lead to endometrial hyperplasia. There is also insulin resistance due to post-receptor problem, not necessarily associated with weight. Also there is association with metabolic syndrome which is Diabetes Mellitus obesit CVS HTN dysplidemia. There is also a decrease in SHBG so there is increase free testosterone.
Signs and symptoms are anvoluation, hyperadrogenism, polycystic ovaries on Transvaginal ultrasound greater than 12 cysts. Patients also have irregular menses, due to high estrogen and lack of progesterone. Leads to high levels of endometrial hyperplasia and the shear weight causes the bleeding. Acanthosis nigricans which is hyperpigmentation in fold of neck and fingers. Infertility or subfertility due to decrease amount of ovulation. Metabolic syndrome is Hypertension, CVS, Dyslipidemia, Obesity, and Diabetes Mellitus type 2. There will also be hirsutism, acne and alopecia.
Still this is a diagnosis of exclusion so must test the thyroid, prolactin levels, primary ovarian failure (high FSH, LH), Late onset CAH so look for 17OH progesterone, Adrogen tumors by looking at androgen index, DHEAS, Cortisol. Then TVUS if greater than 12 cyst in both ovaries. 2hr GTT, GMI, Lipid Profile and BP.
Treatment depends on outcome. Diet and exercise and metformin to address the effects of metabolic syndrome can help regain fertility. First step in management is give Oral Contraceptive to prevent endometrial hyperplasia and prevent cancer, regulates menses. Also estrogen blocks LH and diminishes the androgen to diminish hirsutism and acne. Antiandrogen helps hirsutism acne alopecia. If fertility is the issue than clomiphene and ovarian drilling.
