Additional notes:
Latency is established in different places by the different herpes viruses. Simplex and VZV are latent in the ganglion, but the others are latent in distinct cell populations, as follows: EBV in memory B cells (and rarely, T cells), CMV in T cells,
HHV-6 in B and T cells, HHV-8 in epithelial cells.
Also, diagnostic characteristics for EBV include presence of atypical lymphocytes (usually activated T cells) in blood smears that have the appearance of a '
Dutch skirt'. EBV also causes NON-Hodgkins lymphomas, and lymphoproliferative disease; Oral
Hairy Leukoplakia on the tongue is an indication of EBV in
AIDS patients.
CMV: atypical mono w/ neg monospot (10% of mono); most common viral cause of birth defects (in US) including mental retardation; can also cause colitis, esophagitis, and disseminated infxn.
Primary infxn is usually (80%) asymptomatic.
Culture using
Shell Vial technique or culture from buffy coat, or look for CMV antigen in blood. CMV antigen is almost ubiquitous in urine and is therefore not a useful diagnostic.
Treat with Ganicyclovir and/or Foscarnet.
