Myofascial pain syndrome is a common chronic pain disorder that can affect various parts of the body. Myofascial pain syndrome is characterized by presence of hyperirritable spots located in skeletal muscle called trigger points. A trigger point can be felt as a band or a nodule of muscle with harder than normal consistency. Palpation of trigger points may elicit pain in a different area of the body. This is called referred pain. Referred pain makes diagnosis difficult as the pain mimics symptoms of more well-known common conditions. For example, trigger point related pain in the head and neck region may manifest as tension headache, temporomandibular joint pain, eye pain, or tinnitus.
Symptoms of myofascial pain syndrome include regional, persistent pain, commonly associated with limited range of motion of the affected muscle. The pain is most frequently found in the head, neck, shoulders, extremities, and lower back.
Trigger points are developed as a result of muscle injury. This can be acute trauma caused by sport injury, accident, or chronic muscle overuse brought by repetitive occupational activities, emotional stress or poor posture. A trigger point is composed of many contraction knots where individual muscle fibers contract and cannot relax. These fibers make the muscle shorter and constitute a taut band -- a group of tense muscle fibers extending from the trigger point to muscle attachment. The sustained contraction of muscle sarcomeres compresses local blood supply, resulting in energy shortage of the area. This metabolic crisis activates pain receptors, generating a regional pain pattern that follows a specific nerve passage. The pain patterns are therefore consistent and are well documented for various muscles.
Treatment of myofascial pain syndrome aims to release trigger points and return the affected muscle to original length and strength. Common treatment options include:
- Manual therapy, such as massage, involves application of certain amount of pressure to release trigger points. The outcome of manual therapy strongly depends on the skill level of the therapist.
- The Spray and Stretch technique makes use of a vapor coolant to quickly decrease skin temperature while passively stretching the target muscle. A sudden drop in skin temperature provides a pain relief effect, allowing the muscle to fully stretch, and thus releasing the trigger points.
- Trigger point injections with saline, local anesthetics or steroids are well accepted as effective treatments for myofascial trigger points.
- Dry needling -- insertion of a needle without injecting any solution - is reported to be as effective as injections.

Pain disorder
Pain disorder is chronic pain experienced by a patient in one or more areas, and is thought to be caused by psychological stress. The pain is often so severe that it disables the patient from proper functioning. Duration may be as short as a few days or as long as many years. The disorder may begin at any age, and occurs more frequently in girls than boys. This disorder often occurs after an accident or during an illness that has caused pain, which then takes on a 'life' of its own.
Sub-diagnoses
The DSM-IV-TR specifies three coded subdiagnoses: pain disorder associated with psychological factors, pain disorder associated with both psychological factors and a general medical condition and pain disorder associated with a general medical condition (although the latter subtype is not considered a mental disorder and is coded separately within the DSM-IV-TR). Conditions such as dyspareunia, somatization disorder, conversion disorder, or mood disorders can eliminate pain disorder as a diagnosis. Diagnosis depends on the ability of physicians to explain the symptoms and on psychological influences.

Myofascial pain syndrome is a common chronic pain disorder that can affect various parts of the body. Myofascial pain syndrome is characterized by presence of hyperirritable spots located in skeletal muscle called trigger points. A trigger point can be felt as a band or a nodule of muscle with harder than normal consistency. Palpation of trigger points may elicit pain in a different area of the body. This is called referred pain. Referred pain makes diagnosis difficult as the pain mimics symptoms of more well-known common conditions. For example, trigger point related pain in the head and neck region may manifest as tension headache, temporomandibular joint pain, eye pain, or tinnitus.
Symptoms of myofascial pain syndrome include regional, persistent pain, commonly associated with limited range of motion of the affected muscle. The pain is most frequently found in the head, neck, shoulders, extremities, and lower back.
Trigger points are developed as a result of muscle injury. This can be acute trauma caused by sport injury, accident, or chronic muscle overuse brought by repetitive occupational activities, emotional stress or poor posture. A trigger point is composed of many contraction knots where individual muscle fibers contract and cannot relax. These fibers make the muscle shorter and constitute a taut band -- a group of tense muscle fibers extending from the trigger point to muscle attachment. The sustained contraction of muscle sarcomeres compresses local blood supply, resulting in energy shortage of the area. This metabolic crisis activates pain receptors, generating a regional pain pattern that follows a specific nerve passage. The pain patterns are therefore consistent and are well documented for various muscles.
Treatment of myofascial pain syndrome aims to release trigger points and return the affected muscle to original length and strength. Common treatment options include:
- Manual therapy, such as massage, involves application of certain amount of pressure to release trigger points. The outcome of manual therapy strongly depends on the skill level of the therapist.
- The Spray and Stretch technique makes use of a vapor coolant to quickly decrease skin temperature while passively stretching the target muscle. A sudden drop in skin temperature provides a pain relief effect, allowing the muscle to fully stretch, and thus releasing the trigger points.
- Trigger point injections with saline, local anesthetics or steroids are well accepted as effective treatments for myofascial trigger points.
- Dry needling -- insertion of a needle without injecting any solution - is reported to be as effective as injections.

What Are The Symptoms of TMJ?
The temporomandibular joint disorder is an affliction of the hinge joint (TMJ) that connects the jaw to the skull. The TM joint facilitates all movements of the jaw and therefore, a TMJ disorder might interfere with the most basic functions like eating and talking. To get timely help for this condition, it is essential to get it diagnosed at an early stage. Thus, recognizing the symptoms of TMJ disorder is the first step towards healing it.
Most of the symptoms are likely to occur on or around the jaw area. If you are suffering from TMJ disorder, there are chances that you will feel discomfort in your head, face, mouth, eyes, ears and even back. Contrary to popular belief, toothache is often a symptom rather than a cause of the TMJ disorder. The most common symptoms of TMJ disorder are clicking or grating noises on opening and closing the mouth. These noises occur when there is a slight dislocation of the jaw bones. They herald the onset of a TMJ disorder and must be paid attention to immediately.
For convenience of understanding, the symptoms can be divided into categories, based on which area they are exhibited. These categories are as follows:
1.Symptoms of the face and neck: These symptoms include pain in the jaw, jaw joint, teeth and in the muscles of the cheek. More complicated symptoms would involve uncontrollable tongue and jaw movements, teeth clenching during sleep and locking of the jaw resulting in the inability to open or close the mouth properly. Frequent coughing, intense salivation and voice moderations are among the lesser diagnosed but highly significant symptoms of TMJ disorder.
2.Symptoms of the Head: Aches in the forehead, migraine attacks and sinus pain are some of the common symptoms that occur in the head. Tinnitus, hearing problems, dizziness and lack of balance, clogged ears and blurred vision are other related symptoms.
3.Symptoms throughout the body: Often the pain and discomfort may spread to other parts of the body. These symptoms would involve soreness in the neck and in the shoulders, stiff neck, pain in the lower back and tingling in the tips of fingers and toes. More chronic symptoms include arthritis, reduced motion of limbs and extremely cold sensation in the hands and legs.
Most TMJ disorder symptoms overlap with symptoms of other health conditions. This is why contacting a dentist never occurs to most people suffering from the symptoms of TMJ disorder. The type and intensity of the symptoms may vary greatly from person to person. It is important to contact a health care practitioner at an early stage. One must at all costs avoid bad habits like bruxism and bad body posture. TMJ disorders are caused due to reasons ranging from mental stress to physical injury. It is best, therefore, to adopt a method of treatment that will be wholesome and multi dimensional.
Holistic treatments seek to treat the root cause of the condition thereby eradicating the problem in its entirety and preventing it from happening further. The spine is a complex structure. The function of the structure provides enough flexibility to allow positioning of head and trunk in space for ADLs. The spine houses the neutral structures so it must have strong resistance to excess motion that could damage the spinal cord and nerves. The ligaments and discs serve as passive resistors and dampeners of motion.
A basic overview of the anatomy of the lumbar spine includes 5 lumbar vertebra. The discs are sandwiched between the the vertebra anteriorly. The discs act as shock absorbers for the spine. The alignment is "straight" on AP and lordosis on lateral (due to trapezoidal shape of the discs). The facets posteriorly act as "railroad tracks" and "doorstops". The spinal canal is in a posterior aspect to the vertebra and the nerve roots exit between the vertebra.

Colorado spine surgeon Dr. Donald Corenman lectures to physicians, therapists and chiropractors on how to identify and treat lumbar spinal disorders. The key to rehabilitation is to start to think of how the various spinal structures function and how they can fail. Then, you can think of how this failure will manifest as symptoms. Finally, understanding how different loading positions will affect these structures and understanding how to reverse or ameliorate this disorder.
During his lecture, Dr. Corenman discusses anatomy, physiology, function, alignment, pain generators, biomechanics, degeneration and injury, and treatment protocols for lower back pain. The spine is a complex structure. The function of the structure provides enough flexibility to allow positioning of head and trunk in space for ADLs. The spine houses the neutral structures so it must have strong resistance to excess motion that could damage the spinal cord and nerves. The ligaments and discs serve as passive resistors and dampeners of motion.
