Summary "Pleural mesothelioma": Malignant pleural mesothelioma -- a relatively rare malignant tumor that develops in the mesothelial cells are multipotent and characterized by lesions of all parts of the parietal and visceral pleura and then. In 1767 Lieutaud J. first described the primary tumor of the pleura. Then, in the twentieth century, in 1937, Klemperer D. and Rabin C. gave the first detailed description of pleural mesothelioma. In 1942, experiments Stout A. and Murray M. possible to refine the mesothelial origin of the tumor. But for a long time in the literature described isolated cases of pleural mesothelioma, and only in 1960, Wagner J. described 33 cases of pleural mesothelioma among mine workers mining asbestos. In 1965, Selikoff I. on representative material has shown that exposure to asbestos is the major risk factor for mesothelioma of the pleura. In subsequent years, the main emphasis was given to the various methods of treatment of pleural mesothelioma, but due to the small number of patients, the biological characteristics of the tumor, the lack of randomized trials of diagnosis and treatment of pleural mesothelioma are still relevant to the present. The main etiologic factor in the development of pleural mesothelioma is exposure to asbestos. The most dangerous amphibole fibers (crocidolite, "blue asbestos"): The narrow straight fibers are easy enough to penetrate through the lymph vessels in the pulmonary parenchyma and subpleural space. In addition to asbestos in the development of pleural mesothelioma are also important other chemical carcinogens (silicate, beryllium, liquid paraffin), ionizing radiation (described cases of mesothelioma 20-30 years after radiation therapy of Hodgkin's disease), genetic predisposition, and actively study the role of the virus SV- 40 genes that are expressed in 60% of patients with pleural mesothelioma.- published: 21 Aug 2013

Summary "Symptoms of asbestosis and mesothelioma ": Asbestosis patients typically experience some dust longer than silicosis and average recently approaching 18-20 years. When unfavorable conditions in previous years was significantly shorter length of service. We describe the cases of asbestosis in the length of time in contact with asbestos dust less than 10 and in rare cases even less than 5 years. As with other pneumoconioses, including silicosis, complaints of patients and the clinical manifestations of the disease are most appropriate to moderately expressed bronchitis and emphysema. Compared with subjective complaints of silicosis and other clinical manifestations of asbestosis expressed more prominently. Typically, asbestosis is accompanied by early and significant functional impairment. In this case, a moderately expressed early in the disease syndrome, obstructive disorders gradually joined by diffusion, and then restrictive and restrictive syndrome due to loss of elasticity of lung tissue due to diffuse fibrosis and pleural adhesions. When asbestosis common somatic disorders: weakness, fatigue, pallor with an earthy hue. On the fingers, toes can be found warts ("asbestos warts"). Patients complained of shortness of breath, cough with mucous expectoration and severe pain in the chest (pleural syndrome). When percussion sound is determined by the shade of a box - more than in the upper part, which is usually dominated by emphysema. In the middle and lower lung listened fine moist rales, crackles, pleural friction rub, sometimes dry rales. In the blood is often determined by an accelerated erythrocyte sedimentation rate, usually caused by joining an inflammatory component. When asbestosis, as in the cases of silicosis may be found hyper-gammaglobulinemia, rheumatoid factor, antinuclear antibodies, functional changes of T-lymphocytes. Functional tests of early decline, many indicators of lung ventilation (IAL, pnevmotahimetriya). In more severe cases, a moderately increased residual volume. Violations of diffusion may reduce arterial oxygen saturation. By labeling pneumoconiosis radiographic changes in asbestosis include mainly of type s, t and mainly - u, in other words, a wrong blackouts hilar fibrosis. Coarse reticulation and cellularity usually include irregular rounded and elongated blackout, sometimes abundant, but rarely coalescing into a more or less large fibroid field. Occasionally, these fibrous assemblies have a rounded shape. Conventionally, there are three stages of asbestosis. Stage I is characterized by a thin dense net-like, radiating from the roots to the periphery of the middle and lower lung and pleural-diaphragmatic adhesions. Special diagnostic difficulty is the pleural form of asbestosis, in which the X-ray revealed only pleural changes, mainly the parietal pleura. Similar changes are found in individuals exposed to low concentrations often asbestos dust in the distant past, or after prolonged exposure, even in non-professional environments (living in areas where the asbestos or asbestos companies of the soil). The diagnosis of asbestosis is set mainly on the basis of the professional route, with appropriate clinical and radiological picture. Of particular importance to establish the correct diagnosis with clinical manifestations of bronchial and bronchiolitis in combination with early functional impairments arising ventilation and gas exchange. Prediction of pulmonary asbestosis heavy, as is usually the disease has progredient character, especially with significant functional impairment. Pleural form of asbestosis usually not associated with significant functional impairment and did not cause significant changes in health status.- published: 06 Sep 2013

Summary "Pleural mesothelioma symptoms": Cancer of the pleura (pleural mesothelioma) is a malignant tumor that occurs in people who work with asbestos is 1000 times more frequently than in the general population. The histological diagnosis of malignant pleural mesothelioma in spite of the presence of various histochemical techniques quite complex and in 15 percent of cases to the differential diagnosis of metastatic adenocarcinoma of the impossible. Currently, the diagnosis of pleural mesothelioma is a collection of clinical - especially on the results of visual and endoscopic methods of research - and the pathological-anatomical picture. Delayed onset and late diagnosis often inoperable stage, and the relative resistance to radiation and chemotherapy leads to the fact that pleural mesothelioma is a tumor disease is difficult to treat and have a poor prognosis. With our relative powerlessness in the treatment of these tumors is expected to increase in the incidence in the next decade. Pleural mesothelioma symptoms - Diagnostics The diagnosis of malignant pleural mesothelioma remains a challenge for clinicians and pathologists. The correct diagnosis can be made only with the close collaboration of both doctors as a combination of clinical, radiological, endoscopic and histological studies. Pleural mesothelioma symptoms - History Professional history with clinical suspicion of malignant mesothelioma of the pleura is of great importance. Given the long latency period between exposure and manifestation of the tumor - an average of 35 years - the doctor has to exercise detective skills. The excess of asbestos-containing products is difficult to account, mandatory labeling was introduced only in the late 70s. We have to figure out the working conditions and the potential for toxicity when working with asbestos cement and "weakly related to asbestos fibers," because until the mid 70's the use of these materials has been prescribed to isolate the vent pipes and steel structures. Pleural mesothelioma symptoms - The clinical picture Clinical symptoms are not typical and is of little help in diagnosis. In the early stages of the disease is relatively rare, only 15% of patients are such symptoms as fatigue, weakness, weight loss (4). The most frequent symptoms include dyspnea with subtle start in 75% of cases and the growing harder for chest pain. In contrast to the benign asbestos pleurisy, chest pain does not go away when a pleural effusion. Almost 75% of the common dry cough.- published: 08 Sep 2013

What Does It Really Mean Episode 5 Pleural Effusion
Mesothelioma is a rare form of cancer that develops from cells of the mesothelium, the protective lining that covers many ofthe internal organs of the body. Mesothelioma is most commonly caused by exposure to asbestos.Themost common anatomical site for mesothelioma is the pleura (the outer lining of the lungs and internal chest wall), but it can also arise in the peritoneum (the lining of the abdominal cavity), the pericardium (the sac that surrounds the heart), or the tunica vaginalis (a sac that surrounds the testis). Most people who develop mesothelioma have worked in jobs where they inhaled or ingested asbestos fibers, or were exposed to airborne asbestos dust and fibers in other ways. It has also been suggested that washing clothes of a family member who worked with asbestos increases their risk for developing mesothelioma. Unlike lung cancer, there seems to be no association between mesothelioma and tobacco smoking, but smoking greatly increases the risk of other asbestos-induced cancers. Some people who were exposed to asbestos have collected damages for asbestos-related disease, including mesothelioma. Compensation via asbestos funds or class action lawsuits is an important issue in law practices regarding mesothelioma (see asbestos and the law). Signs and symptoms of mesothelioma include shortness of breath due to pleural effusion (fluid between the lung and the chest wall), chest wall pain, and constitutional signs such as unexplained weight loss. The diagnosis may be suspected based on chest X-ray and CT scan findings, but must be confirmed either by examining serous effusion cytology or with a biopsy (removing a sample of the suspicious tissue). A thoracoscopy (inserting a tube with a camera into the chest) can be used to acquire biopsy material, and allows the introduction of substances such as talc to obliterate the pleural space (a procedure called pleurodesis), preventing more fluid from accumulating and pressing on the lung. Despite treatment with chemotherapy, radiation therapy or sometimes surgery, mesothelioma carries a poor prognosis. Research about screening tests for the early detection of mesothelioma is ongoing. Mesothelioma - a terrible, incurable disease.": Mesothelioma (cancer of the pleura) - a tumor originating from the mesothelial cells or endothelial lymphatic vessels of the pleura. A distinction is limited and diffuse mesothelioma. As with any cancer pathology, it has long been asymptomatic. As early germination vessels appear dull pain unspecified in the corresponding half of the chest. Then the pain becomes more intense, excruciating. Analgesics bring short-term relief. Periodically, there is a low-grade fever. Accumulation of fluid accompanied by a feeling of heaviness in the chest, shortness of breath. Patients take a forced situation in bed with a raised upper half of his body, prefer to sleep on the affected side. Seen from the side of the patient revealed the smoothness of intercostals spaces (much better than in lean patients), the lag in breathing that half of the chest, with percussion - percussion shortening up to the hip, which is projected onto the skin in the form of a characteristic curved lines (line Elissa - Damuazo - Sokolov). Auscultation determined weakened vesicular breathing, often pleural friction rub. Bronchophony voice trembling and weak or undetectable. The intensity of these symptoms depends on the amount of fluid in the pleural cavity and the massiveness of neoplastic lesions of the pleura. On plain film reveals intense homogeneous darkening and was adjacent to the front edge of the chest and the diaphragm and having a trailing border, directed from the top down. Exudate initially clear, yellowish in color, with strands of fibrin. It then becomes hemorrhagic and does not disappear after a puncture. Cytology of pleural fluid sediment are cancer cells. With repeated negative results of the study shows the liquid diagnostic pleurectomy. All patients with suspected pleurisy be hospitalized for a comprehensive examination and treatment. Long-term, gradually decreasing pain occurs when adhesions in the pleura.- published: 16 Sep 2013
