The National Institutes of Health (NIH) is an agency of the United States Department of Health and Human Services and is the primary agency of the United States government responsible for biomedical and health-related research. Its science and engineering counterpart is the National Science Foundation. It comprises 27 separate institutes, centers, and offices which includes the Office of the Director. Francis S. Collins is the current Director.

As of 2003, the NIH was responsible for 28%—about US$26.4 billion—of the total biomedical research funding spent annually in the U.S., with most of the rest coming from industry.

One of the greatest challenges in establishing an academic career is bridging the gap between the beginning stages of such a career as a doctoral student, post-doc, or fellow and the subsequent one as a scientist able to contribute to his/her scientific/clinical area. This gap has been well recognized by both the NIH and by national (and local) scientific organizations and funding mechanisms and policies have been established to facilitate this transition. This workshop will help fellows and young investigators understand the different K award grant mechanisms and will propose strategies to optimize chances of funding, in order to successfully complete the challenging transition to established investigator. In a time of tight federal budgets it is imperative that each applicant submit an outstanding application. This workshop is very timely for both trainees interested in submitting an application and for potential mentors. Dr. Mark Roltsch, a former NHLBI Program Officer and Scientific Review Officer, created this workshop to share his insight of years of career development awards review and program management as well as his knowledge of the internal workings of NIH grants for young investigators in an effort to enhance the attendees knowledge of what is need to write a successful career development grant and how to avoid some common pitfalls.

It has been defined by the National Heart, Lung and Blood Institute as a common chronic disorder of the airways that is complex and characterized by variable and recurring symptoms, airflow obstruction, bronchial hyperresponsiveness (bronchospasm), and an underlying inflammation. The interaction of these features of asthma determines the clinical manifestations and severity of asthma and the response to treatment. Public attention in the developed world has recently focused on asthma because of its rapidly increasing prevalence, affecting up to one in four urban children. Asthma is caused by a complex interaction of environmental and genetic factors that researchers do not yet fully understand.[55] These factors can also influence how severe a persons asthma is and how well they respond to medication. As with other complex diseases, many environmental and genetic factors have been suggested as causes of asthma, but not all studies posing such claims have been verified by further studies. In addition, as researchers detangle the complex causes of asthma, it is becoming more evident that certain environmental and genetic factors may affect asthma only when combined. Asthma is an airway disease that can be classified physiologically as a variable and partially reversible obstruction to air flow, and pathologically with overdeveloped mucus glands, airway thickening due to scarring and inflammation, and bronchoconstriction, the narrowing of the airways in the lungs due to the tightening of surrounding smooth muscle. Bronchial inflammation also causes narrowing due to edema and swelling caused by an immune response to allergens. During an asthma episode, inflamed airways react to environmental triggers such as smoke, dust, or pollen. The airways narrow and produce excess mucus, making it difficult to breathe. In essence, asthma is the result of an immune response in the bronchial airways. The airways of asthmatics are "hypersensitive" to certain triggers, also known as stimuli (see below). (It is usually classified as type I hypersensitivity.) In response to exposure to these triggers, the bronchi (large airways) contract into spasm (an "asthma attack"). Inflammation soon follows, leading to a further narrowing of the airways and excessive mucus production, which leads to coughing and other breathing difficulties. Bronchospasm may resolve spontaneously in 12 hours, or in about 50% of subjects, may become part of a 'late' response, where this initial insult is followed 312 hours later with further bronchoconstriction and inflammation. The normal caliber of the bronchus is maintained by a balanced functioning of these systems, which both operate reflexively. The parasympathetic reflex loop consists of afferent nerve endings which originate under the inner lining of the bronchus. Whenever these afferent nerve endings are stimulated (for example, by dust, cold air or fumes) impulses travel to the brain-stem vagal center, then down the vagal efferent pathway to again reach the bronchial small airways. Acetylcholine is released from the efferent nerve endings. This acetylcholine results in the excessive formation of inositol 1,4,5-trisphosphate (IP3) in bronchial smooth muscle cells which leads to muscle shortening and this initiates bronchoconstriction.

IDAHO FALLS - A local teenager is fighting an extremely rare disease often referred to as "Childhood Alzheimer's." Kristen Wells, 17, was diagnosed with Niemann-Pick Type C (NPC) five years ago. It's a disease that causes her body cells to attack each other, according the National Center of Biotechnology Information. In most parts of her body, the cells are able to reproduce and regenerate the cell damage. The exception is her brain, where the cells don't reproduce, which causes a gradual loss of neurological function. About one in 150,000 people are diagnosed with NPC. It is always fatal and the earlier the onset of the disease, the shorter the life expectancy of the child. When Kristen was 12, she began showing signs of dyslexia. The Idaho Falls teenager slowly started losing things and everyday tasks became challenges. "She was in one of her special education classes and one of her teachers noticed she knew how to do the math but she couldn’t get the order right," said Lori Wells, Kristen's mother. Eventually, Kristen's parents took her to local doctor, who referred her to Primary Children’s Medical Center in Salt Lake City. An MRI was ordered and the results weren’t good. "When they came back with the MRI results, the cerebellum in her brain had degenerated," Wells said. "(The) person who read it said (the MRI) looked like a person who’s been a drunk all their life." After nine months of tests, doctors diagnosed Kristen with the genetic disease. Kristen’s parents are both carriers but they’re both adopted and had no idea the disease runs in both of their families. "You never know you’re a carrier until two carriers come together and you get a child with the disease," Wells said. Statistically speaking, four of the Wells six child should have NPC but Kristen, the youngest, is the only one with the disease. Because there’s no cure, she and her mother travel to the National Institute of Health in Washington D.C. every month for drugs and participate in studies. Kristen’s medication costs about 17 hundred dollars a day, resulting in $620,000 every year for the medication. Fortunately, for the Wells, insurance covers most of the cost but it does take a financial toll on the family. "I look at that and just laugh, because there’s a house payment every day of the month," Wells said. A few years ago, the Make a Wish Foundation heard about Kristen and her desire to go Italy with her parents and all her siblings. The wish came true and for a week, she left her trials behind for the trip of a lifetime. Now, even as the disease progresses, Kristen’s parents said she teaches them important lessons every day. "It makes you extremely more patient because you don’t hurry to do anything," Wade Wells, Kristen's dad, said. The Wells family are thankful for all they've learned and the blessings Kristen has brought their family. "You just take everything as it comes," Lori Wells said. "Why make yourself miserable over something you don't really have control over? We just choose to be happy."

Szára, who later worked for the US National Institutes of Health, had turned his attention to DMT after his order for LSD from the Swiss company Sandoz Laboratories was rejected on the grounds that the powerful psychotropic could be dangerous in the hands of a communist country.[7] DMT can produce powerful psychedelic experiences including intense visuals, euphoria and hallucinations.[92] DMT is generally not active orally unless it is combined with a monoamine oxidase inhibitor (MAOI) such as a reversible inhibitor of monoamine oxidase A (RIMA), for example, harmaline.[3] Without an MAOI, the body quickly metabolizes orally administered DMT, and it therefore has no hallucinogenic effect unless the dose exceeds monoamine oxidase's metabolic capacity. Other means of ingestion such as vaporizing, injecting, or insufflating the drug can produce powerful hallucinations for a short time (usually less than half an hour), as the DMT reaches the brain before it can be metabolized by the body's natural monoamine oxidase. Taking a MAOI prior to vaporizing or injecting DMT prolongs and potentiates the effects.[92]

Dr. Heit's grant will be used to study the genome-wide association of venous thrombosis (blood clots in the veins). Deep vein thrombosis is a blood clot in a vein — a blood vessel that returns oxygen-depleted blood to your heart. These clots are different from those that form in an artery — a blood vessel that carries oxygen-rich blood from your heart to the rest of your body. Risk factors for developing DVT include surgery, hospitalization for medical illness, major trauma (especially a long bone fracture of the leg), cancer and neurological disorders that cause leg weakness. When a blood clot (thrombus) develops in a vein, the danger is that it will break loose and travel to your lungs, where it can become stuck. This serious condition is known as a pulmonary embolism and, in some cases, the result is sudden death. It's estimated that each year about 1 million Americans have a DVT or pulmonary embolism episode; about one-third are fatal. After an additional year of postdoctoral study at the NYU Medical School, he joined the NYU Chemistry Department in 1961. He has held a Career Development Award from the National Institutes of Health and has been awarded (with physicist Paul Davies) the Trotter Prize in Complexity, Information and Inference for 2004. In addition to his research, Professor Shapiro has written four books and several magazine articles for the general public. The books include: 1. "Life Beyond Earth", co-author with Gerald Feinberg, (Morrow 1980). This book advanced a very positive view about the possibilities for life elsewhere in the Universe. For example, it discussed the possibility of life in underground niches on Mars, in oceans beneath the frozen surfaces of the satellites of Jupiter, and in various environments within Titan, a satellite of Saturn. Today, these sites rank at the top of NASA's agenda, in searching for life within our Solar System. In addition, it described a number of exotic possibilities for life based on substances other than carbon or water. Timothy Ferris, commented in a front-page review in the New York Times Book Review: "Informative and authoritative yet venturesome and readable, 'Life Beyond Earth' is one of the best books on Earth about life elsewhere." The New Yorker observed: "For people who like to rerad about solid scientific speculation, this is an exhilarating, informative, and ultimately liberating book." 2. "Origins, a Skeptic's Guide to the Creation of Life on Earth", (Simon & Schuster, 1986). This work is described elsewhere in this web site. 3. "The Human Blueprint", (St. Martin's Press, 1991). This work described the current and future course of the Human Genome Project. It anticipated the use of DNA testing to resolve the Thomas Jefferson-Sally Hemings paternity question, and the Anastasia controversy, as well as the use of DNA analysis to track the historical movement of human populations. It was an alternate Book-of-the Month Club Selection for Sept. 1991, and in German translation, this book was selected by the jury "Bild am Wissenschaft" as the "Wissenschaftbuch des Jahres" (Science Book of the Year). 4. "Planetary Dreams", (Wiley, 1999). This work is described elsewhere in this web site. Professor Shapiro has commented: "obviously, I have developed a taste for very large scientific topics". When he is not involved in research, lecturing or writing, he enjoys running, hiking, wine-tastings, raquetball, and travel.
