  • OFA Evaluation

Eye Examination by a boarded ACVO Ophthalmologist- recommend annual exams until age 10

  • Results registered with OFA - OR
  • Results registered with CERF

Progressive Retinal Atrophy (PRA) DNA Test

  • DNA based test from an approved laboratory

GM1 GANGLIOSIDOSIS

  • OFA registry of GM1 DNA test from NYU

Congenital Cardiac Database (Optional)

  • OFA evaluation with exam performed by a Cardiologist
  • or OFA evaluation with exam performed by a Specialist

Elbow Dysplasia (Optional)

  • OFA Evaluation

Autoimmune thyroiditis (Optional)

  • OFA evaluation from an approved laboratory

Sebaceous Adenitis (Optional)

  • Sebaceous Adenitis - OFA evaluation

Patellar Luxation (Optional)

  • OFA Evaluation

Hip Dysplasia (Optional)

  • PennHIP Evaluation

Juvenile Dilated Cardiomyopathy - Univ of Pennsylvania - (Optional)

*Note – In addition to the breed specific requirements above, a CHIC requirement across all participating breeds is that the dog must be permanently identified via microchip or tattoo in order to qualify for a CHIC number.
