We will be discussing the pathology, pathophysiology, causes and etiology, signs and symptoms, lab findings and management of this skin disorder.
ERYTHEMA MULTIFORME
Erythema multiforme is the mildest form of the epidermal necrolysis condition. Erythema Multiforme has a very characteristic target lesion that is very similar to urticarial. It is differentiated from erythema multiforme by the fact that urticarial disappears after 24 hours whereas erythema multiforme doesn’t.
Causes and etiology – Herpes, mycoplasma, drugs allergies may imitate erythema multiforme
History – Generally it is not preceded by Prodrome such as cough or sore throat, but they may have fever with a history of prior attacks.
Examination – normally found on face and extremities. Also exhibits the Koebner phenomenon. Sometimes the central area can blister. Mucocutaneous lesion may also be seen.
Course – usually last 2 weeka nd then there is resolution without sequelae, however recurrence is common.
Management – hospitalize if there is impaired feeding, may develop to SJS, and if there are constitutional symptoms.
EPIDERMAL NECROLYSIS
STEVEN JOHNSON SYNDROME (SJS)
Defined as involvement of less than 10percent of body surface area.
TOXIC EPIDERMYLOSIS NECROLYSIS (TEN)
Defined as involvement of more than 30 percent of body surface area.
CAUSES – High risk drugs – Sulfonamide, anti-convulsants, allopurinol, NSAIDs, NSAIDs usually taken 8 weeks prior. Also viral infections, immunizations, mycoplasma, GVHD.
Clinical – skin peels off for 5-7 days and plateaus for days to week and this is when there is sepsis and failure. Starts with prodromal symptoms such as fever, headaches, rhinitis. Cutaneous begins with face to upper trunk and then to upper limbs. Lesions starts as erythematous macules that then become confluent. These patients are Nikolsky’s sign positive meaning skin will rub off with minor pressure. Mucocuteneous lesions consists of pain on swallowing, eye pain. Extracutaneous symptoms related to pulmonary system (ARDS, Bronchiectasis), Esophageal to colon necrosis causing diarrhea, melena, malabsorption. Kidneys, nails, vulva, vagina involvement may also be present.
Prognosis scoring system is known as the SCORTEN. Age greater than 40, heart rate greater than 120, presence of cancer, BSA greater than 10 percent, urea greater than 20mmol, HCO3 greater than 20mmol and glucose greater than 14mmol. If more than 5 than 90 percent mortality.
Management – Oxygen and ABG. Check glucose, urea, and HCO3. First begin by stopping the drug which is causing the allergy. Admit to ICU or Burns wards and give fluids and electrolytes. If infection than give anti-biotics. Mouth rinse with anti-septic wash every day. Give emollients, anti-biotics and vitamin A for 24 hours. Medications such as corticosteroids, IVIG, cyclosporine A, plasmaphoresis and hemodialsis

We will be discussing the pathology, pathophysiology, causes and etiology, signs and symptoms, lab findings and management of this skin disorder. ERYTHEMA MULTIFORME Erythema multiforme is the mildest form of the epidermal necrolysis condition. Erythema Multiforme has a very characteristic target lesion that is very similar to urticarial. It is differentiated from erythema multiforme by the fact that urticarial disappears after 24 hours whereas erythema multiforme doesn’t. Causes and etiology – Herpes, mycoplasma, drugs allergies may imitate erythema multiforme History – Generally it is not preceded by Prodrome such as cough or sore throat, but they may have fever with a history of prior attacks. Examination – normally found on face and extremities. Also exhibits the Koebner phenomenon. Sometimes the central area can blister. Mucocutaneous lesion may also be seen. Course – usually last 2 weeka nd then there is resolution without sequelae, however recurrence is common. Management – hospitalize if there is impaired feeding, may develop to SJS, and if there are constitutional symptoms. EPIDERMAL NECROLYSIS STEVEN JOHNSON SYNDROME (SJS) Defined as involvement of less than 10percent of body surface area. TOXIC EPIDERMYLOSIS NECROLYSIS (TEN) Defined as involvement of more than 30 percent of body surface area. CAUSES – High risk drugs – Sulfonamide, anti-convulsants, allopurinol, NSAIDs, NSAIDs usually taken 8 weeks prior. Also viral infections, immunizations, mycoplasma, GVHD. Clinical – skin peels off for 5-7 days and plateaus for days to week and this is when there is sepsis and failure. Starts with prodromal symptoms such as fever, headaches, rhinitis. Cutaneous begins with face to upper trunk and then to upper limbs. Lesions starts as erythematous macules that then become confluent. These patients are Nikolsky’s sign positive meaning skin will rub off with minor pressure. Mucocuteneous lesions consists of pain on swallowing, eye pain. Extracutaneous symptoms related to pulmonary system (ARDS, Bronchiectasis), Esophageal to colon necrosis causing diarrhea, melena, malabsorption. Kidneys, nails, vulva, vagina involvement may also be present. Prognosis scoring system is known as the SCORTEN. Age greater than 40, heart rate greater than 120, presence of cancer, BSA greater than 10 percent, urea greater than 20mmol, HCO3 greater than 20mmol and glucose greater than 14mmol. If more than 5 than 90 percent mortality. Management – Oxygen and ABG. Check glucose, urea, and HCO3. First begin by stopping the drug which is causing the allergy. Admit to ICU or Burns wards and give fluids and electrolytes. If infection than give anti-biotics. Mouth rinse with anti-septic wash every day. Give emollients, anti-biotics and vitamin A for 24 hours. Medications such as corticosteroids, IVIG, cyclosporine A, plasmaphoresis and hemodialsis -

Sin embargo, aún cuando estaba subtitulado en español, se me hacia complicado leer y ver con atención el video, puesto que éste es muy dinámico y entretenido. We will be discussing the pathology, pathophysiology, causes and etiology, signs and symptoms, lab findings and management of this skin disorder.
ERYTHEMA MULTIFORME
Erythema multiforme is the mildest form of the epidermal necrolysis condition. Erythema Multiforme has a very characteristic target lesion that is very similar to urticarial. It is differentiated from erythema multiforme by the fact that urticarial disappears after 24 hours whereas erythema multiforme doesn’t.
Causes and etiology – Herpes, mycoplasma, drugs allergies may imitate erythema multiforme
History – Generally it is not preceded by Prodrome such as cough or sore throat, but they may have fever with a history of prior attacks.
Examination – normally found on face and extremities. Also exhibits the Koebner phenomenon. Sometimes the central area can blister. Mucocutaneous lesion may also be seen.
Course – usually last 2 weeka nd then there is resolution without sequelae, however recurrence is common.
Management – hospitalize if there is impaired feeding, may develop to SJS, and if there are constitutional symptoms.
EPIDERMAL NECROLYSIS
STEVEN JOHNSON SYNDROME (SJS)
Defined as involvement of less than 10percent of body surface area.
TOXIC EPIDERMYLOSIS NECROLYSIS (TEN)
Defined as involvement of more than 30 percent of body surface area.
CAUSES – High risk drugs – Sulfonamide, anti-convulsants, allopurinol, NSAIDs, NSAIDs usually taken 8 weeks prior. Also viral infections, immunizations, mycoplasma, GVHD.
Clinical – skin peels off for 5-7 days and plateaus for days to week and this is when there is sepsis and failure. Starts with prodromal symptoms such as fever, headaches, rhinitis. Cutaneous begins with face to upper trunk and then to upper limbs. Lesions starts as erythematous macules that then become confluent. These patients are Nikolsky’s sign positive meaning skin will rub off with minor pressure. Mucocuteneous lesions consists of pain on swallowing, eye pain. Extracutaneous symptoms related to pulmonary system (ARDS, Bronchiectasis), Esophageal to colon necrosis causing diarrhea, melena, malabsorption. Kidneys, nails, vulva, vagina involvement may also be present.
Prognosis scoring system is known as the SCORTEN. Age greater than 40, heart rate greater than 120, presence of cancer, BSA greater than 10 percent, urea greater than 20mmol, HCO3 greater than 20mmol and glucose greater than 14mmol. If more than 5 than 90 percent mortality.
Management – Oxygen and ABG. Check glucose, urea, and HCO3. First begin by stopping the drug which is causing the allergy. Admit to ICU or Burns wards and give fluids and electrolytes. If infection than give anti-biotics. Mouth rinse with anti-septic wash every day. Give emollients, anti-biotics and vitamin A for 24 hours. Medications such as corticosteroids, IVIG, cyclosporine A, plasmaphoresis and hemodialsis

No dia 19 de abril de 2013, nosso bebê, de apenas um ano de idade, foi encaminhado a Santa Casa de Misericórdia em Sorocaba para tratar de pneumonia. No terceir...
No dia 19 de abril de 2013, nosso bebê, de apenas um ano de idade, foi encaminhado a Santa Casa de Misericórdia em Sorocaba para tratar de pneumonia. No terceiro dia de internação ele não apresentava melhoras. Surgiu a suspeita de haver contraído gripe suína. Não havia tempo para esperar o resultado. Imediatamente começou o tratamento contra pneumonia e também contra a gripe suína. Após alguns dias, começou a melhorar e ao fim de duas semanas voltou para casa. Era um sábado, dia 04 de maio de 2013.
No dia 05 ele amanheceu com o olho esquerdo com secreção. Sua mucosa oral estava inflamada, sua face manchada, seu corpinho inchado. Sua mamãe levou-o ao Pronto Atendimento da Zona Norte de Sorocaba.
A médica de plantão nem olhou pra ele. Ao ouvir da mamãe que o olho estava com secreção, rapidamente emitiu uma receita com remédio para conjuntivite.
Foi piorando a cada dia, até que no dia 08 de maio, sua mamãe levou-o de volta à Santa Casa. Imediatamente foi internado.
Fizeram exames. Primeiro diagnóstico: Meningite Bacteriana. Após 24 horas novo diagnóstico: Síndrome de Steven Johnson, alergia causada por medicamentos ou vírus.
Mas qual foi o remédio que causou isto???????
Não sabemos ainda. O resultado a biopsia não saiu, assim como não temos o resultado se foi realmente gripe suína.
Ele piorou a cada dia, sendo transferido para o GPACI de Sorocaba para a UTI. Chegamos a conseguir uma vaga no Hospital Universitário de SP, mas era perigoso o transporte.
Infelizmente o quadro piorou. Estava vivendo apenas pelos aparelhos. No horário de visita do GPACVI, às 16:30, ao vê-lo, sabíamos que a luta estava acabando. Seus rins não estavam respondendo, seus pulmões estavam cheios de secreção, que saiam pelas narinas em forma de espuma. Seu corpinho estava totalmente queimado, e assim era por dentro também. Já havia tido uma parada cardíaca.
A noite, os médicos chamaram a mim, que sou a vovó materna, e a mãezinha. Avisara-nos: Ele não teve passar desta noite.
Choramos muito, mas decidimos que respeitaríamos a vontade do Senhor. Oramos, pedindo ao Senhor que se este era Seus planos, que fosse breve, para que ele parasse de sofrer.
Em meia hora veia resposta. Ele havia tido mais 3 paradas cardíacas e partido. Era dia 18 de maio de 2013, quando nosso pequeno guerreiro parou de lutar e foi ser ninado pelo Senhor.
E agora? A duvida que fica é, será que podia ter sido evitado tudo isto se a médica do PA tivesse diagnosticado quando lá fomos dia 05? Não abemos e acho que nunca saberemos.
Porque este vídeo foi feito? Foi a maneira que encontrei para lidar com a dor imensa pela perda de nosso bebê. Ao mesmo tempo, é um alerta para que estejamos mais preparados para lidar com doenças tão pouco conhecidas e letais.
Agradecemos imensamente a todas as igrejas que se mobilizaram em oração, e em especial a Pra Cristiane, da Igreja do Evangelho Quadrangular da Vila Hortência em Sorocaba, que esteve conosco nos apoiando, e também a minha aluna de inglês Priscila, que fez de tudo que podia para nos ajudar a arrumar a vaga em SP.

Lo encontré en internet y me pareció muy interesante. Sin embargo, aún cuando estaba subtitulado en español, se me hacia complicado leer y ver con atención el video, puesto que éste es muy dinámico y entretenido. Entonces pensé en la necesidad de una traducción hablada al español. Me atreví a hacerlo yo mismo y aquí está, aunque no es del mismo tipo del que ya he hecho para éste canal. Pronto haré uno nuevo.

Lo encontré en internet y me pareció muy interesante. Sin embargo, aún cuando estaba subtitulado en español, se me hacia complicado leer y ver con atención el video, puesto que éste es muy dinámico y entretenido. Entonces pensé en la necesidad de una traducción hablada al español. Me atreví a hacerlo yo mismo y aquí está, aunque no es del mismo tipo del que ya he hecho para éste canal. Pronto haré uno nuevo.
