The patient was 53 yrs male weighing 104Kg and came with hyperglycemia and toxemia. He was started on titrated doses of human act rapid insulin and antibiotics. Pus was sent for culture and sensitivity. He was investigated and prepared for surgery. Excision of the carbuncle was under taken under intubation general anesthesia. Patient has been discharged now. His blood sugar level is under control and TLC has fallen to near normal. He is getting dressing once daily and is on insulin and antibiotics.
Diabetes is a notorious disease, it lowers the immunity of the patient and makes the patient prone to infections. One such infection is the development of Carbuncle due to Staphylococci. It is usually present at the nape of the neck or on the back but it can develop rarely on other sites to in the body. This lady developed this on her upper arm. It is often accompanied by hyperglycemia and septicemia which have to be controlled too.

In my opinion, carbuncles are kind of like super boils... and I don't want one. They just seem so painful and hard to treat. What do you think? What are your thoughts about carbuncles, boils and cysts?

Carbuncle
A carbuncle is a cluster of boils, draining pus onto the skin. It is usually caused by bacterial infection, most commonly with Staphylococcus aureus or Streptococcus pyogenes, which can turn lethal. However, the presence of a carbuncle is actually a sign that the immune system is working. The infection is contagious and may spread to other areas of the body, or other people; those living in the same residence may develop carbuncles at the same time.
Signs and symptoms
A carbuncle is made up of several skin boils. The infected mass is filled with fluid, pus and dead tissue. Fluid may drain out of the carbuncle, but sometimes the mass is so deep that it cannot drain on its own. Carbuncles may develop anywhere, but they are most common on the back and the nape of the neck.
The carbuncle may be the size of a pea or as large as a golf ball. It may be red and irritated, and might hurt when touched. It may also grow very fast and have a white or yellow center. It may crust or spread to other skin areas. Sometimes other symptoms may occur, such as fatigue, fever and a general discomfort or sick feeling. Itching may occur before the carbuncle develops.

Pus was sent for culture and sensitivity. He was investigated and prepared for surgery. Excision of the carbuncle was under taken under intubation general anesthesia. Patient has been discharged now. His blood sugar level is under control and TLC has fallen to near normal. He is getting dressing once daily and is on insulin and antibiotics.
